针刺肌肉起止点与息风通络汤联合应用对于脑卒中后痉挛性偏瘫患者的肌体功能恢复、中医临床症状及生活质量的影响

摘　　要：目的探讨针刺肌肉起止点联合息风通络汤治疗脑卒中后痉挛性偏瘫的临床疗效。方法选择2020年6月—2021年12月我科收治的72例脑卒中后痉挛性偏瘫的患者,根据随机数字表法分为观察组(36例)与对照组(36例)。观察组采用针刺肌肉起止点联合息风通络汤治疗,对照组采用普通针刺联合息风通络汤治疗。对比观察两组患者中医证候积分(半身不遂、舌强语謇、肢体麻木)、日常生活活动能力量表(ADL)评分、改良Ashworth痉挛状态量表评分、临床痉挛指数(CSI)评分、NIHSS神经功能缺损评分量表评分。结果两组治疗后半身不遂、舌强语謇、肢体麻木的中医证候均较治疗前有显著好转(P<0.01),且观察组半身不遂、舌强语謇、肢体麻木的中医证候评分低于对照组(P<0.01)。两组治疗后ADL评分、改良Ashworth痉挛状态量表评分、临床痉挛指数(CSI)评分、NIHSS评分均较治疗前好转(P<0.05),且观察组ADL评分、改良Ashworth痉挛状态量表评分、临床痉挛指数(CSI)评分、NIHSS评分均优于对照组(P<0.05)。结论针刺肌肉起止点联合息风通络汤治疗脑卒中后痉挛性瘫痪患者可显著改善患者中医临床症状和日常生活能力,降低患者的肌肉痉挛、肌张力和神经功能缺损,对提高患者生活质量具有重要临床意义。Objective To explore the clinical effect of acupuncture at the starting and ending points of muscle combined with Xifeng Tongluo Decoction on spastic hemiplegia after stroke.Methods Seventy-two patients with spastic hemiplegia after stroke treated in Rehabilitation Hospital of Hexi District from June 2020 to December 2021 were randomly assigned into an observation group(n=36)and a control group(n=36).The observation group was treated with acupuncture at the starting and ending points of muscle combined with Xifeng Tongluo Decoction,while the control group with general acupuncture combined with Xifeng Tongluo Decoction.The scores of TCM syndromes(hemiplegia,stiff tongue impeding speech,limb numbness),activities of daily living(ADL)scale,modified Ashworth scale of spasticity,clinical spasticity index(CSI),and National Institutes of Health Stroke Scale(NIHSS)were compared between the two groups.Results The TCM syndromes in both groups were alleviated after treatment(P<0.01),with the scores in the observation group lower than those in the control group(P<0.01).Furthermore,the scores of ADL scale,modified Ashworth spasticity scale,CSI,and NIHSS in both groups were recovered after treatment(P<0.05),and the observation group outperformed the control group(P<0.05).Conclusion Acupuncture at the starting and ending points of muscle combined with Xifeng Tongluo Decoction can significantly alleviate the clinical symptoms,improve daily living activities,and mitigate the muscle spasm,muscle tension,and nerve functional defect of the patients with spastic hemiplegia,which is of clinical significance to improve the quality of life of patients.
