摘  要:目的:探讨高频重复经颅磁刺激联合穴位按摩对卒中后睡眠障碍(PSSD)患者的干预效果及对脑内神经递质和血清褪黑素水平的影响。方法:选取2019年4月—2021年4月脑卒中后睡眠障碍患者97例,采用抽签法分为对照组48例和观察组49例。对照组给予穴位按摩干预,观察组在对照组基础上增加高频重复经颅磁刺激,对比两组患者睡眠质量、脑内神经递质和血清褪黑素水平。结果:干预前,两组睡眠质量、脑内神经递质和血清褪黑素水平比较,差异无统计学意义(P>0.05);干预后,观察组入睡时间、睡眠质量、睡眠效率、睡眠时间、睡眠障碍、催眠药物和日间功能障碍、匹兹堡睡眠质量指数量表(PQSI)总分、多巴胺(DA)水平低于对照组,5-羟色胺(5-HT)和干预后3 d, 10 d, 30 d时褪黑素水平高于对照组,(P<0.05)。结论:高频重复经颅磁刺激联合穴位按摩可以有效改善卒中后睡眠障碍患者的脑内神经递质水平及褪黑素水平,进而提高睡眠质量。Objective: To explore the intervention effect of high-frequency repetitive transcranial magnetic stimulation combined with acupoint massage on post-stroke sleep disorder(PSSD) patients, as well as the effects of brain neurotransmitters and serum melatonin levels.Methods: A total of 97 patients with post-stroke sleep disorders in department from April 2019 to April 2021 were selected as the research objects, 48 cases in the control group and 49 cases in the observation group were divided by lottery.The control group was given physiological-behavioral sleep nursing intervention.On the basis of the control group, the observation group added high-frequency repetitive transcranial magnetic stimulation and acupoint massage.The sleep quality, brain neurotransmitter and serum melatonin levels were compared between the two groups.Results:Before intervention,there was no statistical significance in sleep quality,brain neurotransmitter level,and serum melatonin level between the two groups(P>0.05).Sleep disturbance,hypnotic drugs and daytime dysfunction,Pittsburgh Sleep Quality Index Scale(PQSI)total score,dopamine(DA)level lower than control group,5-hydroxytryptamine(5-HT),the protein level of melanogenesis at 3d,10d,30dwas higher than that of the control group(P<0.05).Conclusion:High-frequency and high-frequency repetitive transcranial magnetic stimulation combined with acupoint massage can effectively improve the levels of neurotransmitters and melatonin in the brain of patients with post-stroke sleep disorders,thereby improving sleep quality.
