摘  要:目的 探讨Angio平面波微细血流成像(AP)技术评价脑梗死患者颈动脉斑块内新生血管(IPN)评分及斑块内微血管血流(IMVF)分级的应用价值,以及其与急性前循环脑梗死(acACI)发病的关系。方法 纳入acACI 50例和非急性脑梗死(nACI)90例,比较两组患者的一般资料、颈动脉斑块常规彩超特征和IPN评分及IMVF分级的差异。结果 两组患者在颈动脉斑块回声特点上存在显著差异(P<0.05);AP检查结果表明,acACI组IPN的比例、IPN评分和IMVF分级均高于nACI组,差异均有统计学意义(P<0.001)。acACI组IMVF分级与IPN数目、部位、评分之间存在正相关,相关系数r分别为0.602、0.634和0.618(P<0.001)。结论 急性前循环脑梗死患者颈动脉的IPN评分和IMVF分级更高,提示IPN可能与其发生有关;AP超声技术为IPN的早期筛查提供了一种无创、简便、快捷的检查手段。Objective To evaluate the application value of carotid artery intraplaque neovascularization(IPN) score and intraplaque microvascular flow(IMVF) grade in patients with cerebral infarction by angio planewave ultrasensitive imaging(AngioPLUS, AP) technique, and its relationships with anterior circulation acute cerebral infarction(acACI). Methods A total of 50 patients with acACI and 90 patients with non-acute cerebral infarction(nACI) were enrolled to compare the differences in general information, conventional color Doppler ultrasound characteristics of carotid plaque, IPN score and IMVF grading between the two groups. Results There were significant differences in carotid plaque echo between two groups(P<0.05). AP examination showed that the proportion of IPN, IPN score and IMVF grade in acACI group were higher than those in nACI group, and the differences were statistical significance(P<0.001). There was a positive correlation between IMVF grading and IPN number, location and score in acACI group, with correlation coefficients r of 0.602, 0.634 and 0.618 respectively(P<0.001). Conclusions The IPN score and IMVF grade of the carotid plaque with acACI are higher;suggesting that IPN may be related to the occurrence of acACI. AP ultrasound technology provides a non-invasive, simple and fast examination method for detecting of IPN early.
