摘 要:目的:探讨高度浓缩生长因子结合可吸收骨材料(Bio-oss骨粉)在老年前牙种植中的应用效果。方法:采用随机数字表法,将我院64例于2018年11月-2020年5月开展前牙种植手术患者分为对照组(n=32)、观察组(n=32)。对照组术中添加Bio-oss骨粉,观察组术中添加高度浓缩生长因子+Bio-oss骨粉,对两组种植成功率,术后种植体周围状况,手术前、后龈沟液血管内皮生长因子(VEGF)、骨形成蛋白-2(BMP-2)水平予以比较。结果:12个月内种植成功率对照组(90.63%)与观察组(96.88%)比较,无明显差异(P>0.05);术后6个月,唇侧骨板厚度、出血指数、探诊深度、红色美学指数观察组相较于对照组,情况明显更好(P<0.05);术后4周、术后8周,相较于对照组,观察组龈沟液VEGF、BMP-2水平提升更明显(P<0.05)。结论:在老年前牙种植中应用高度浓缩生长因子结合可吸收骨材料,能提升龈沟液VEGF、BMP-2水平,促进种植体周围软、硬组织恢复。Objective:To investigate the application effect of highly concentrated growth factor combined with resorbable bone material(Bio-oss bone meal)in anterior dental implants in elderly patients.Methods:Using the random number table method,64 patients in our hospital who carried out elderly anterior teeth implantation from Nov 2018 to May 2020 were divided into the control group(n=32)and the observation group(n=32).In the control group,Bio-oss bone meal was added during the operation in the control group,while highly concentrated growth factor+Bio-oss bone meal were added during the operation.The implantation success rate,postoperative peri-implant conditions,the levels of vascular endothelial growth factor(VEGF)and bone morphogenetic protein-2(BMP-2)in gingival crevicular fluid before and after surgery were compared between the two groups.Results:There was no significant difference in the success rate between the control group(90.63%)and the observation group(96.88%)within 12 months(P>0.05).At 6 months after operation,the labial bone plate thickness,bleeding index,probing depth and red aesthetic index in the observation group were compared with the control group,the situation is obviously better(P<0.05).At 4 weeks and 8 weeks after operation,compared with the control group,the levels of VEGF and BMP-2 in the gingival crevicular fluid in the observation group were significantly increased(P<0.05).Conclusion:The application effect of highly concentrated growth factor combined with resorbable bone material in anterior dental implants in elderly patients can increase the levels of VEGF and BMP-2 in gingival crevicular fluid,and promote the recovery of soft and hard tissues around the implant.
