摘　　要：血管内机械取栓是近年来急性缺血性卒中治疗领域的重要进展。既往已有充分的循证医学证据表明,血管内取栓治疗对发病24h内的前循环大血管闭塞性卒中安全有效。与前循环大血管闭塞性卒中相比,急性基底动脉闭塞单纯药物治疗的死亡率和致残率更高,而取栓治疗对这部分患者的有效性和安全性一直缺乏高级别的循证医学证据支持。随着近期后循环机械取栓临床研究结果的陆续报道,尤其是《新英格兰医学杂志》最新发表的BAOCHE以及ATTENTION研究阳性结果的问世,急性基底动脉闭塞的取栓治疗有了新的进展。因此,本文将对急性基底动脉闭塞的取栓治疗研究进展及未来展望作一述评,旨在为临床实践中急性基底动脉闭塞的机械取栓治疗提供参考。Endovascular mechanical thrombectomy is an important progress in the treatment of acute ischemic stroke in recent years.Sufficient evidence proved that endovascular thrombectomy is safe and effective for acute ischemic stroke caused by large vessel occlusion stroke in the anterior circulation within 24 hours from symptom onset.Compared with the anterior circulation ischemic stroke,the mortality and disability rate of acute basilar-artery occlusion treated with medical care alone are higher,and the effectiveness and safety of endovascular thrombectomy for these patients have been lack of high-level evidence.With the recent reports of mechanical thrombectomy for posterior circulation stroke,especially the latest positive results of BAOCHE and ATTENTION studies published in The New England Journal of Medicine,the thrombectomy treatment of acute basilar artery occlusion has made new progress.In this editorials,we will review the research progress and future prospects of thrombectomy for acute basilar artery occlusion,in order to provide reference for mechanical thrombectomy for acute basilar artery occlusion in clinical practice.
