摘 要:目的探讨能谱计算机断层扫描(CT)物质定量技术在辅助诊断甲状腺相关性眼病(TAO)方面的应用价值。方法纳入2021年4月—2022年3月于中国中医科学院眼科医院确诊为TAO的患者44例(88只眼)作为试验组,选择同期同医院就诊的同年龄段健康体检者32例(64只眼)作为对照组。对2组受试者均行能谱CT检查,分别测得眼球突出度、球后脂肪分析和球后有效原子序数的数据,并做统计分析。结果(1)眼球突出度、球后脂肪分析和有效原子序数:试验组眼球突出度较对照组大(t=12.366,P=0.000),球后脂肪分析较对照组高(t=7.105,P=0.000),有效原子序数较对照组高(t=8.486,P=0.000),差异均有统计学意义。(2)相关性分析:眼球突出度与眼球后脂肪分析(r=0.504,P=0.000)、眼球后有效原子序数(r=0.311,P=0.003)均呈正相关,均有统计学意义。结论能谱CT可提供脂肪含量分析和有效原子数值,二者可考虑作为TAO诊断的判定指标。OBJECTIVE To investigate the application value of spectral computed tomography(CT)quantitative technique in the auxiliary clinical diagnosis of thyroid associated ophthalmopathy(TAO).METHODS From April 2021 to March 2022,a total of 44 cases(88 eyes)diagnosed as TAO in Eye Hospital of China Academy of Chinese Medical Sciences were included as the experimental group,and 32 healthy subjects(64 eyes)of the same age at the same hospital were selected as the control group during the same period.All subjects in the two groups underwent orbital spectral CT examination,and the data of exophthalmos,postbulbar fat analysis and effective atomic number(Effective-Z)were measured and statistically analyzed.RESULTS(1)Exophthalmos,fat analysis,and Effective-Z:The exophthalmos of the experimental group was larger than that of the control group(t=12.366,P=0.000).The postbulbar fat analysis was higher than that of the control group(t=7.105,P=0.000).Also,the Effective-Z was higher than that of the control group(t=8.486,P=0.000).The differences were all statistically significant.(2)Correlation analysis:There were positive correlations between postbulbar fat analysis and exophthalmos(r=0.504,P=0.000),Effective-Z and exophthalmos(r=0.311,P=0.003).There were all statistically significant.CONCLUSIONS Spectral CT can provide fat analysis and Effective-Z,which can be considered as diagnostic indicators for TAO.
