牙周基础治疗联合根管治疗牙周牙髓联合病变的疗效及对炎性因子的影响研究

摘　　要：目的分析牙周牙髓联合病变患者予以牙周基础治疗联合根管治疗的疗效及对炎性因子的影响。方法84例牙周牙髓联合病变患者,以随机抽签法分为对照组和观察组,每组42例。对照组患者单一使用根管治疗,观察组在对照组基础上联合牙周基础治疗。比较两组患者的临床疗效及治疗前后血清炎性因子[白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)及超敏C反应蛋白(hs-CRP)]、菌斑指数(PLI)及牙龈指数(GI)。结果观察组患者治疗总有效率95.24%高于对照组的78.57%,差异有统计学意义(P<0.05)。治疗后,观察组IL-1β(4.23±0.63)ng/L、TNF-α(6.11±0.82)ng/L、hs-CRP(7.58±1.06)mg/L及PLI(0.73±0.20)分、GI(1.03±0.32)分均低于对照组的(5.16±0.71)ng/L、(7.48±1.02)ng/L、(9.36±1.32)mg/L、(1.06±0.32)分、(1.49±0.37)分,差异有统计学意义(P<0.05)。结论牙周牙髓联合病变患者应用牙周基础治疗联合根管治疗利于改善患者炎症反应,提升临床疗效,值得临床进一步推广。Objective To analyze the efficacy of basic periodontal therapy combined with root canal therapy in the treatment of combined periodontal-endodontic lesions and its influence on inflammatory factors.Methods A total of 84 patients with combined periodontal-endodontic lesions were randomly divided into control group and observation group,with 42 cases in each group.Patients in the control group received root canal therapy alone,while patients in the observation group received basic periodontal therapy on the basis of the control group.Both groups were compared in terms of clinical efficacy,serum inflammatory factors[interleukin1β(IL-1β),tumor necrosis factor-α(TNF-α)and high-sensitivity C-reactive protein(hs-CRP)],plaque index(PLI)and gingival index(GI)before and after treatment.Results The total effective rate 95.24%in the observation group was higher than 78.57%in the control group,and the difference was statistically significant(P<0.05).After treatment,the IL-1β(4.23±0.63)ng/L,TNF-α(6.11±0.82)ng/L,hs-CRP(7.58±1.06)mg/L,PLI(0.73±0.20),GI(1.03±0.32)points in the observation group were lower than(5.16±0.71)ng/L,(7.48±1.02)ng/L,(9.36±1.32)mg/L,(1.06±0.32)points,(1.49±0.37)points in the control group,and the differences were all statistically significant(P<0.05).Conclusion The combination of root canal therapy and basic periodontal therapy for patients with combined periodontal-endodontic lesions can improve the inflammatory response of and improve the clinical efficacy,which is worthy of further clinical promotion.
