癌症疼痛患者止痛治疗依从性的影响因素分析

摘　　要：目的:探究癌症疼痛患者止痛治疗依从性的影响因素。方法:选取2018年8月-2020年7月都江堰市人民医院门诊收治的75例晚期癌症患者作为研究对象,采用调查问卷的方式对患者开展止痛治疗依从性调查,分析癌症疼痛患者止痛治疗依从性及癌症疼痛患者止痛治疗依从性的影响因素。结果:癌症疼痛患者治疗依从性较差,未按时用药占比最高,达100.00%;其次为减量用药占比58.67%,不服药占比46.67%,漏服药占比40.00%,癌痛时才用药占比36.00%。癌症疼痛患者止痛治疗依从性的主要影响因素为成瘾恐惧,占比最高达100.00%;其次为对止痛药物误解占比80.00%,对治疗效果有所担忧占比73.33%,医疗费用高占比66.67%,担心药物安全性占比57.33%,其他占比13.33%。结论:成瘾恐惧、对治疗效果有所担忧、医疗费用高、对止痛药物误解、担心药物安全性是癌症疼痛患者止痛治疗依从性的主要影响因素,建议予以患者健康宣教、心理疏导以及制定合理的止痛方案等,消除患者疑虑,提高其治疗自信心,确保治疗安全性与有效性,持续提升患者止痛治疗依从性。Objective:To explore the influencing factors of analgesic treatment compliance in patients with cancer pain.Methods:Seventy-five patients with advanced cancer who were admitted to the People's Hospital of Dujiangyan from August 2018 to July 2020 were selected as the study subjects.Questionnaire was used to investigate the analgesic treatment compliance of patients with cancer pain.The analgesic treatment compliance of patients with cancer pain and the influencing factors were analyzed.Results:The treatment compliance of patients with cancer pain was poor,the highest proportion was not taking drugs on time,accounting for 100.00%;followed by reducing drug dosage,accounting for 58.67%,not taking drugs,accounting for 46.67%,missing drugs,accounting for 40.00%,and taking drugs only when cancer pain occurs,accounting for 36.00%.The main influencing factors for the compliance of cancer pain patients with analgesic treatment were addiction fear,accounting for 100.00%,followed by misunderstanding of analgesic drugs,accounting for 80.00%,worrying about the treatment effect,accounting for 73.33%,high medical expenses,accounting for 66.67%,worrying about the safety of drugs,accounting for 57.33%,and others,accounting for 13.33%.Conclusion:The addiction fear,treatment effect concerns,high medical expenses,misunderstanding of analgesic drugs,worrying about the safety of drugs are the main influencing factors for the compliance of cancer pain patients with analgesic treatment,it's suggested to perform health education and psychological counseling for patients,formulate reasonable analgesic scheme,eliminate patient's anxiety,improve the self-confidence with treatment,ensure the treatment safety and effectiveness,and continuously improve analgesic treatment compliance of patients.
