摘  要:目的 观察安宫牛黄丸联合亚低温对心脏骤停心肺复苏(CPR)后患者脑氧代谢率、氧化应激损伤的影响。方法 将我院急诊室抢救的118例心脏骤停心肺复苏后患者随机分对照组与观察组各59例。对照组予以亚低温等西医基础治疗,观察组在对照组治疗基础上予以安宫牛黄丸治疗。统计两组临床疗效,记录两组急性生理学与慢性健康状况评分Ⅱ(APACHEⅡ)、格拉斯哥昏迷评分(GCS昏迷)、序贯器官衰竭评估(SOFA)评分,脑氧代谢率(CMRO)值,超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、丙二醛(MDA)、S-100蛋白(S-100)、基质金属蛋白酶-9(MMP-9)水平。结果 治疗后,观察组预后良好率高于对照组(P<0.05);与治疗前比较,两组APACHEⅡ、SOFA评分均显著下降,GCS昏迷评分显著增高(P<0.05),观察组APACHEⅡ、SOFA评分显著低于对照组,GCS昏迷评分显著高于对照组(P<0.05);与治疗前比较,两组CMRO、SOD、GSH-Px水平均显著增高,MDA水平显著下降(P<0.05),观察组CMRO、SOD、GSH-Px水平显著高于对照组,MDA水平显著低于对照组(P<0.05);与治疗前比较,两组S-100、MMP-9水平均显著下降(P<0.05),观察组S-100、MMP-9水平均显著低于对照组(P<0.05)。结论 安宫牛黄丸联合亚低温显著提高心脏骤停CPR后患者脑氧代谢率,降低氧化应激损伤,提高预后。Objective To observe the effect of Angong Niuhuang Pills combined with mild hypothermia on cerebral oxygen metabolism rate and oxidative stress injury in patients with cardiac arrest after cardiopulmonary resuscitation(CPR).Methods A total of 118 patients with cardiac arrest after cardiopulmonary resuscitation who were rescued in the emergency room of our hospital were selected and randomly divided into control group and study group,with 59 cases in each group.Control group was given basic western medicine treatment such as mild hypothermia,and study group was given Angong Niuhuang Pills on the basis of control group. The curative effects of the two groups were calculated,and the acute physiology and chronic health status scoreⅡ(APACHEⅡ),Glasgow coma score(GCS coma),sequential organ failure assessment (SOFA)score,cerebral metabolic rate of oxygen(CMRO)value,superoxide Dismutase(SOD),glutathione peroxidase(GSH-Px),malondialdehyde(MDA),S-100 protein(S-100),matrix metalloproteinase-9(MMP-9)levels were recorded.Results The good prognosis rate of study group was higher than that of control group( P<0.05);compared with before treatment,the APACHEⅡ and SOFA scores of both groups after treatment were significantly decreased,which were significantly lower in study group than in control group( P<0.05),the GCS coma score was significantly increased( P<0.05),which was significantly higher in study group than in control group( P<0.05);CMRO,SOD and GSH-Px levels in both groups were significantly increased,which were higher in study group than in control group,MDA level was significantly decreased( P<0.05);which were significantly lower in study group than in control group( P<0.05);S-100 and MMP-9 levels in both groups were significantly decreased( P<0.05),which were significantly lower in study group than in control group( P<0.05).Conclusion Angong Niuhuang Pills combined with mild hypothermia could significantly improve the cerebral oxygen metabolism rate,reduce oxidative stress injury and improve prognosis in patients
