摘　　要：目的:探讨颈内动脉发育异常的临床和高分辨磁共振管壁成像(HR-VWI)特征。方法:收集6例经影像学确诊的颈内动脉发育异常患者,分析其临床和影像学资料,并复习总结相关文献。结果:6例患者中4例为男性,2例为女性,年龄31~64岁。就诊原因分别为头晕、头痛或肢体无力4例,一过性晕厥1例,静止性震颤伴行动迟缓1例。6例均为单侧,其中3例颈内动脉发育不良患者于HR-VWI上表现为颈内动脉起始处以远全程管径纤细,管壁未见明显增厚及强化;3例颈内动脉发育不全患者颈内动脉起始处均可见显影,病例1和5起始处以远延续为线样软组织样结构,分别止于海绵窦段和颈段,病例6起始处以远缺如。颅内伴动脉夹层、动脉瘤、发育不良、异常血管网等多种血管异常。结论:颈内动脉发育异常在HR-VWI上有一定特征性,且HR-VWI能够有效评估颅内侧支代偿及其合并的血管异常。Aim:To investigate the clinical and high-resolution magnetic resonance vessel wall imaging(HR-VWI) features of the internal carotid artery(ICA) congenital anomalies.Methods:Clinical and imaging data of 6 patients with ICA congenital anomalies were analyzed retrospectively,and the related literature was reviewed.Results:Six patients aged from 31 to 64 years presented with dizziness,headache or weakness in limbs(4 cases),transient syncope(1 case),quiescent tremor and slow movement(1 case),respectively.All the 6 cases were unilateral,among whom,ICA displasia was observed in 3 cases,showing smooth diffuse narrowing of the ICA lumen sparing its initial portion without wall thickening or enhancement on HR-VWI.ICA aplasia was found in 3 cases,out of which,one only showed the initial portion on HR-VWI,the other two showed the normal initial portion and a tiny band of distal portion terminating at the C1,C4 portion,respectively.Besides,intracranial artery dissection,aneurysm,dysplasia,and abnormal vascular network were observed.Conclusion:ICA congenital anomalies have certain characteristics on HR-VWI,and HR-VWI could effectively evaluate intracranial collateral pathways and vascular abnormalities.
