无肾移植病史肾淋巴增生性疾病误诊为肾透明细胞癌1例

摘　　要：肾淋巴增生性疾病是一种较为罕见的疾病,其多发生在有肾移植病史的患者。我院收治了1例73岁男性患者,首因消化系统疾病入消化内科治疗,住院中完善腹部彩超时发现右肾占位,CTU检查后考虑肾透明细胞癌(cTNM,Ⅲ期),后转入我科后行腹腔镜根治性肾切除术,术后病理经免疫组化证实为右侧肾脏淋巴增生性病变。追问患者既往未曾行手术治疗,无免疫系统疾病病史,未服用过免疫抑制剂。基于此病例较为罕见,现通过回顾性分析相关文献,总结此类患者的临床诊治。Renal lymphoproliferative disease is a rare disease that occurs mostly in patients with a history of kidney transplantation.Recently,a 73-year-old man was admitted to our hospital.He was admitted to the Department of Gastroenterology for the first time due to digestive system diseases.During the hospitalization,a space-occupying right kidney was found by ultrasound imaging of the abdomen.After completing the CTU examination,clear cell renal cell carcinoma(cTNM,stageⅢ)was considered.Later,he was transferred to our department and underwent laparoscopic radical nephrectomy.The postoperative pathology was confirmed by immunohistochemistry:right renal lymphoproliferative lesions.The patients had no history of surgery,immune system diseases or immunosuppressive agents.Based on the rarity of this case,the clinical diagnosis and treatment of such patients are summarized through a retrospective analysis of relevant literature.
