摘 要:目的:探讨肌电图在神经根型颈椎病中的诊断价值,为临床中神经根型颈椎病的诊断与治疗提供电生理依据。方法:选择2019年1月至2021年1月在秦皇岛市第一医院确诊的神经根型颈椎病患者150例,性别不限,年龄30~70岁。根据是否伴有肌力下降分为伴肌力下降组和不伴肌力下降组,采用肌电图记录神经和肌肉的生物电活动,比较两组患者电生理结果。结果:伴肌力下降组周围神经运动传导、正中神经F波、针电极肌电图异常率高于不伴肌力下降组(P<0.05)。异常针电极肌电图中以C_(6)、C_(7)神经根支配肌神经源性损害所占比例最大(73.6%),与颈椎MRI结果提示C_(5~6)、C_(6~7)椎间盘突出或狭窄所占比例最大(60.2%)一致。结论:肌电图检查对伴有肌力下降的神经根型颈椎病患者具有更高的诊断价值,可根据肌电图的不同表现形式对神经根的损害部位、损害程度进行评估。Objective:To explore the diagnostic value of electromyography in cervical spondylotic radiculopathym(CSR),and provide electrophysiological basis for the diagnosis and treatment of CSR.Methods:A total of 150 patients with CSR diagnosed in the First Hospital of Qinhuangdao City from January 2019 to January 2021 were enrolled in the study,both of sexes,aged 30-70 years.According to whether there was a decrease in muscle strength,patients were divided into the group with decreased muscle strength and the group without decreased muscle strength.Electromyography was used to record the bioelectrical activity of nerve and muscle,and the electrophysiological results were compared between the two groups.Results:The abnormal rates of peripheral nerve motor conduction,median nerve F wave and needle electrode electromyography in the group with decreased muscle strength were higher than those in the group without decreased muscle strength(P<0.05).In abnormal needle electrode electromyography,C_(6) and C_(7) nerve root innervated muscle neurogenic damage accounted for the largest proportion(73.6%),which is consistent with the results of cervical MRI indicating that C_(5~6) and C_(6~7) intervertebral disc herniation or stenosis accounted for the largest proportion(60.2%).Conclusion:Electromyography has higher diagnostic value for cervical spondylotic radiculopathy with decreased muscle strength.It can evaluate the damage location and degree of nerve root according to different manifestations of electromyography.
