摘　　要：目的探讨小型桥小脑角脑膜瘤的手术治疗效果并总结手术经验。方法回顾性分析2016年1月—2020年12月期间苏州大学第一附属医院神经外科收治并手术治疗的73例小型桥小脑角脑膜瘤患者,设为手术组。选取同期立体定向放射治疗小型桥小脑角脑膜瘤的5篇文献复习,统计文献中患者的临床资料,设为放疗组。将两组的临床结果,影像学结果和后续治疗进行比较。结果手术组的症状恶化率,肿瘤增大率和后续治疗率与放疗组相仿。SimpsonⅡ级切除是肿瘤增大的独立危险因素。结论与立体定向放射治疗相比,手术治疗小型桥小脑角脑膜瘤在缓解症状、抑制肿瘤进展方面疗效相仿。同时手术治疗能够获得准确的病理检查结果,指导后续治疗。因此小型桥小脑角脑膜瘤首先可以考虑手术治疗。Objective To discuss the surgical treatment of small cerebellopontine angle meningioma and summarize the surgical experience.Methods 73 patients with small cerebellopontine angle meningioma admitted by the Department of Neurosurgery,the First Affiliated Hospital of Soochow University from January 2016 to December 2020 were analyzed retrospectively and set as the operation group.Five articles about simultaneous stereotactic radiotherapy for small cerebellopontine angle meningioma were reviewed.The clinical data of patients in the literature were collected and set up as the radiotherapy group.The symptom deterioration rate,tumor enlargement rate and further treatments rate of the two groups were compared.Results The symptom deterioration rate,tumor enlargement rate and further treatments rate of surgery group were similar to those in the radiotherapy group.SimpsonⅡresection grade was an independent risk factor for tumor enlargement.Conclusions Compared with stereotactic radiotherapy,surgery for small cerebellopontine angle meningiomas has better outcomes in relieving symptoms and inhibiting tumor progression.Surgery could obtain the exact pathological examination for guiding the further treatment.Therefore,surgical treatment of small cerebellopontine angle meningiomas can be considered as the first choice.
