摘　　要：目的探讨Sirdah值对地中海贫血的预测价值。方法分析190439例健康体检者红细胞参数,计算Sirdah值=平均红细胞体积-红细胞计数-3×Hb。随机对100例红细胞参数异常者行铁蛋白及Hb电泳检测,绘制ROC曲线分析Sirdah值对地中海贫血的预测价值。结果红细胞参数异常者1660例。与红细胞参数正常者比较,红细胞参数异常者的Hb、平均红细胞体积和Sirdah值较低,红细胞计数较高(P<0.05)。100例红细胞参数异常者中,地中海贫血患者84例。与非地中海贫血者比较,地中海贫血者红细胞计数、Hb较高,平均红细胞体积、Sirdah值较低(P<0.05)。Sirdah值预测地中海贫血的AUC为0.946[95%CI(0.910~1.000)],当Sirdah值取最佳诊断界值为24.1时,对应的预测地中海贫血的灵敏度为100%,特异度为76.19%。结论红细胞参数异常者和地中海贫血患者的Sirdah值均较低,Sirdah值对地中海贫血有较高的预测价值。Objective To investigate the predictive value of Sirdah value for thalassemia.Methods The RBC parameters of 190439 people with health examination were analyzed.The Sirdah value was calculated as the formula of MCV-RBC-3×Hb.The ferritin and hemoglobin electrophoresis were detected in 100 cases with abnormal RBC parameters.The predictive value of Sirdah value for thalassemia was analyzed by ROC curve.Results There were 1660 patients with abnormal RBC parameters.The Hb,MCV and Sirdah value were lower and RBC was higher in the patients with abnormal RBC parameters than those with normal RBC parameters(P<0.05).Among 100 patients with abnormal RBC parameters,84 patients with thalassemia had higher RBC and Hb,lower MCV and Sirdah value than those without thalassemia(P<0.05).The AUC of Sirdah value for predicting thalassemia was 0.946[95%CI(0.910-1.000)].When the best cut-off value of Sirdah value was 24.1,the corresponding sensitivity for diagnosing thalassemia was 100%,and the specificity was 76.19%.Conclusion Sirdah value of the patients with abnormal RBC parameters and thalassemia is lower.Sirdah value has higher predictive value for thalassemia.
