摘 要:目的:探讨彩色多普勒超声联合MRI动态增强在诊断乳腺硬化性腺病中的应用价值。方法:回顾性分析经手术及病理证实的88例SA患者共96枚病灶的临床资料,分别将彩色超声、MRI动态增强、联合诊断结果与病理结果进行对照研究。结果:SA病灶的超声和MR影像表现均无特异性,超声显示Adler彩色血供程度分级以0-I级者居多;MR强化方式多样,TIC曲线以I型、II型为主;超声和MRI各自诊断SA(含合并恶性成分)的准确率、误诊率、漏诊率分别为57.29%、39.58%、3.13%(55/96、38/96、3/96)和59.38%、36.46%、4.17%(57/96、35/96、4/96),单一方法诊断能力有限。然而两者联合诊断后,准确率、误诊率、漏诊率为79.17%、20.83%、0(76/96、20/96、0/96),误、漏诊率降低,诊断准确性提高。结论:超声与MRI联合应用,可优势互补,对减少SA的误、漏诊率,提高诊断的准确性有较大的价值。Objective:To explore the application value of color doppler ultrasound combined with MRI dynamic enhancement in diagnosing sclerosing adenosis of breast.Methods:The clinical data of 96 lesions in 88 patients with SA confirmed by surgery and pathology were analyzed retrospectively.The results of color doppler ultrasound,MRI dynamic enhancement and combined diagnosis were compared with pathological results.Results:There was no specific image features of SA in ultrasound and MRI.The Adler color blood supply grades of most SA lesions in ultrasound were 0-I.There were various ways of MR enhancement,and the TIC curve was mainly type I and type II.The accuracy rate,misdiagnosis rate and missed diagnosis rate of ultrasound and MRI in diagnosing SA lesions(including malignant components)were 57.29%,39.58%,3.13%(55/96,38/96,3/96)and 59.38%,36.46%,4.17%(57/96,35/96,4/96)respectively.The diagnostic ability of single method was limited.However,after combined diagnosis of the two methods,the accuracy rate,misdiagnosis rate and missed diagnosis rate were 79.17%,20.83%and 0(76/96,20/96 and 0/96).The misdiagnosis rate and missed diagnosis rate decreased and the diagnostic accuracy improved.Conclusion:The combined application of ultrasound and MRI can complement each other and has great value in reducing the misdiagnosis and missed diagnosis rate and improving the diagnostic accuracy of SA.
