摘　　要：目的通过对慢性根尖周炎初次根管治疗与根管再治疗中产黑色素类杆菌(BPB)进行培养鉴定及统计分析,为临床治疗、用药及院内感染控制提供理论依据。方法从2019年10月至2021年5月医院接收的患者中随机抽取40例慢性根尖周炎未经治疗的患者(初次根管治疗组),以及40例慢性根尖周炎治疗后根尖周再次感染的患者(根管再治疗组),比较两组根管治疗中BPB及粪肠球菌的检出情况及不同临床症状患者的BPB检出情况。结果两组BPB检出率比较,差异无统计学意义(P>0.05);根管再治疗组粪肠球菌检出率高于初次根管治疗组,差异有统计学意义(P<0.05)。根管再治疗组疼痛肿胀、叩痛、根管内渗出患者的BPB检出率均高于初次根管治疗组,差异有统计学意义(P<0.05);两组窦道形成症状患者的BPB检出率比较,差异无统计学意义(P>0.05)。结论BPB在初次感染根管与再感染根管中均有定植,其中,再感染根管中BPB与患牙疼痛肿胀、叩痛、根管内渗出存在一定相关性。Objective With the identification and statistical analysis of blackpigmented bacteria in chronic periapical periodontitis with primary root canal therapy and root canal retreatment,the theoretical basis for clinical treatment,medication and hospital infection control was provided.Methods With the random selection of 40 untreated patients with chronic periapical periodontitis(primary root canal therapy group)and 40 patients with periapical reinfection after treatment of chronic periapical periodontitis(root canal retreatment group),among the patients received by the hospital from October 2019 to May 2021,the detection results of BPB and enterococcus faecalis,and the BPB in patients with different clinical symptoms were compared between the two groups with root canal therapy.Results There was no significant difference in the detection rate of BPB between the two groups(P>0.05);The detection rate of enterococcus faecalis in the root canal retreatment group was higher than that in the primary root canal treatment group,and the difference was statistically significant(P<0.05).The detection rate of BPB in patients with pain swelling,percussion pain and root canal exudation in the root canal re treatment group was higher than that in the primary root canal treatment group,and the difference was statistically significant(P<0.05);There was no significant difference in the detection rate of BPB between the two groups of patients with sinus formation symptoms(P>0.05).Conclusion With BPB cloning in both primary and reinfected root canals,there is a certain correlation between BPB in the reinfected root canal and the pain,swelling,tapping pain and root canal exudation of the affected tooth.
