支撑喉镜下低温等离子射频消融技术治疗早期声门型喉癌的疗效及对嗓音功能的影响分析

摘　　要：目的 探讨支撑喉镜下低温等离子射频消融技术治疗早期声门型喉癌的疗效及对嗓音功能的影响。方法 选取40例早期声门型喉癌患者开展研究,时间2012年6月至2022年6月。按照手术方案的差异将患者分成两组,其中20例行支撑喉镜下低温等离子射频消融治疗,纳入观察组;另20例患者支撑喉镜下以钬激光治疗,纳入对照组。对比两组治疗效果、手术情况、嗓音功能、并发症情况。结果 观察组总体有效率达90.00%(18/20),对照组总体有效率达85.00%(17/20),两组对比差异不显著(P> 0.05)。观察组手术时间长于对照组,住院时间、愈合时间短于对照组(P <0.05),而两组术中出血量对比差异不显著(P> 0.05)。术前两组嗓音功能客观评估对比差异不显著(P> 0.05);术后观察组基频高于对照组,基频微扰、振幅微扰低于对照组(P <0.05)。观察组术后并发症发生率为10.00%(2/20),对照组术后并发症发生率为25.00%(5/20),两组对比差异不显著(P> 0.05)。结论 支撑喉镜下低温等离子射频消融术较钬激光治疗早期声门型喉癌的疗效无显著差异,但前者切除时创伤更小,术后恢复更快,对嗓音功能的影响也较小,同时也能降低并发症的发生率。Objective To investigate the effect of low-temperature plasma radiofrequency ablation under supporting laryngoscope in the treatment of early glottic laryngeal cancer and its impact on voice function.Methods Forty patients with early glottic laryngeal cancer were selected for Time June 2012-June 2022.The patients were divided into two groups according to the difference of surgical scheme,among which 20 patients were treated with low-temperature plasma radiofrequency ablation under supporting laryngoscope,and were divided into observation group;another 20 patients were treated with holmium laser under supporting laryngoscope.The treatment effect,operation,voice function and complications were compared between the two groups.Results The total effective rate of the observation group was 90.00%(18/20) and that of the control group was 85.00%(17/20).There was no significant difference between the two groups(P>0.05).The operation time in the observation group was longer than that in the control group,the hospitalization time and healing time in the observation group were shorter than those in the control group(P0.05).There was no significant difference in objective evaluation of voice function between the two groups before operation(P>0.05).After operation,the fundamental frequency of the observation group was higher than that of the control group,and the fundamental frequency perturbation and amplitude perturbation were lower than those of the control group(P0.05).Conclusion There is no significant difference in the low-temperature plasma radiofrequency ablation under laryngoscope compared with holmium laser under supporting laryngoscope was treated with early glottic carcinoma,but the former is less trauma during resection and recovery faster after surgery,the impact on voice function is smaller,and the occurrence of complications can be controlled to a certain extent.
