摘 要:目的 探讨传统细胞培养法和PCR结合导流杂交法对解脲支原体(UU)和人型支原体(MH)的检出率、灵敏度、检测速度等差异,判断PCR结合导流杂交法在生殖道感染检测中应用的可行性。方法 选取2020年12月至2021年2月在首都医科大学附属北京友谊医院(通州院区)妇产科就诊的患者301例,将收集的子宫颈脱落细胞分成2份,1份用于细胞培养法进行UU、MH培养鉴定,药敏试剂盒进行病原体的检测;另外1份样本通过PCR结合导流杂交法进行MH、UU[含解脲脲原体(Uu)和微小脲原体(Up1/3/6/14)共5种亚型]、生殖支原体(MG)、淋病奈瑟菌(NG)、沙眼衣原体(CT)和单纯疱疹病毒Ⅱ型(HSVⅡ)共10种微生物的检测。结果 细胞培养法检出率:MH 6.64%(20/301)、UU 47.18%(142/301)。PCR结合导流杂交法检测结果如下:MH 17.28%(52/301)、UU(含Uu和Up1/3/6/14):53.82%(162/301)、MG 6.98%(21/301)、NG 2.33%(7/301)、CT 10.30%(31/301)、HSVⅡ6.98%(21/301)。2种方法对UU检测的一致性标本占76.08%(229/301),47例差异标本的Sanger测序结果与PCR结合导流杂交法检测结果一致,25例与细胞培养法一致;MH一致性的标本占85.38%(257/301),31例差异标本的Sanger测序结果与PCR结合导流杂交法检测结果一致,13例与细胞培养法一致。结论 PCR结合导流杂交法与细胞培养法对MH、UU的检测具有较高的一致性,均具有较高的特异度和准确率。PCR结合导流杂交法在检测耗时、阳性检出率、检测通量和检测指标数量上均有较大优势,对NG、CT、MG和HSVⅡ的结果输出以及协同感染的检出,对后续的治疗均有重要的参考意义。Objective To evaluate the feasibility of PCR combined with diversion hybridization for the detection of Ureaplasma urealyticum(UU)and Mycoplasma hominis(MH)by comparing the differences in detection rate,sensitivity and detection speed between traditional culture method and PCR combined with diversion hybridization.Methods A total of 301 patients were selected in the Department of Obstetrics and Gynecology of Beijing Friendship Hospital(Tongzhou District)from December 2020 to February 2021. The collected cervical exudated cells were divided into two parts,one for the detection of pathogens by UU and MH culture. Another sample was used to detect 10 microorganisms in MH,UU(including ureaplasma UU,ureaplasma micron UP1/3/6/14,a total of 5 subtypes),Mycoplasma genitalium(MG),Neisseria gonorrhoeae(NG),Chlamydia trachomatis(CT)and zonal herpes virus type Ⅱ(HSV Ⅱ )by PCR combined with flow hybridization.Results The positive rate of MH was 6.64%(20/301)and 47.18%(142/301)by cell culture method. The results of PCR combined with diversion hybridization were as follows:MH:17.28%(52/301);UU(including Uu,Up1/3/6/14):53.82%(162/301);Mycoplasma genitalium(MG):6.98%(21/301);Neisseria gonorrhoeae(NG):2.33%(7/301);Chlamydia trachomatis(CT):10.30%(31/301);Herpes simplex virus(HSV)Ⅱ:6.98%(21/301).About 76.08%(229/301)of the samples were consistent for UU. The Sanger sequencing results of 47 different samples were consistent with the PCR combined with diversion hybridization detection results,and 25 samples were consistent with the culture method. About 85.38%(257/301)of the samples were consistent for MH.The results of PCR combined with diversion hybridization were consistent with the results of Sanger sequencing in 31 different samples,and the results of culture method and sequencing method in 13 different samples were consistent.Conclusions PCR combined with diversion hybridization has high consistency,specificity and accuracy.
