两种角膜切口缝合在白内障手术中的应用效果对比

摘　　要：目的 对比分析3.5 mm自闭式无缝线透明角膜切口与2.0 mm透明角膜不缝合自闭式切口在白内障手术中的应用效果。方法选取本院2018年4月至2019年4月收治的96例白内障手术患者,均行超声乳化联合人工晶体植入术,根据手术切口方式的不同将其分为A组与B组,每组各48例。A组采用3.5 mm自闭式无缝线透明角膜切口,B组采用2.0 mm透明角膜不缝合自闭式切口,对比两组视力、手术源性散光、屈光度水平以及术后并发症。结果 B组术后1、4、12周的视力均高于A组,手术源性散光均小于A组,屈光度水平均大于A组,上述数据组间比较差异均有统计学意义(t=4.085、3.194、3.479、4.601、5.494、3.891、3.986、5.411、5.584,均P<0.05)。B组术后1、4、12周屈光度改变率均低于A组,术后并发症发生率低于A组,两组比较差异均有统计学意义(χ^(2)=4.800、4.042、4.631、4.800,均P<0.05)。结论 2.0 mm透明角膜不缝合自闭式切口在白内障手术中的应用效果优于3.5 mm自闭式无缝线透明角膜切口,值得临床应用。Objective To compare and analyze the effect of 3.5 mm self-closing sutureless transparent corneal incision and 2.0 mm clear corneal non-suture self-closing incision in cataract surgery. Methods A total of 96 cases of cataract patients who underwent phacoemulsification combined with intraocular lens implantation were selected from April 2018 to April 2019 were divided into group A and group B according to different surgical incisions,48 cases in each group. The patients in group A were used 3.5 mm self-closing sutureless transparent corneal incision, and in group B were used 2.0 mm transparent corneal non-suturing self-closing incision. The visual acuity, surgical astigmatism, diopter level and postoperative complications in two groups were compared. Results The visual acuity of group B were higher than that of group A, the surgical astigmatism of group B were lower than that of group A, and the diopter of group B were higher than that of group A at 1, 4 and 12 weeks after operation, the differences of all data between the groups were statistically significant(t=4.085, 3.194, 3.479, 4.601, 5.494, 3.891, 3.986, 5.411, 5.584, all P<0.05);and the diopter change rate at 1, 4 and 12weeks after operation of group B was lower than that of group A, and the incidence of complications of group B was lower than that of group A, the differences between the groups were statistically significant(χ^(2)=4.800, 4.042, 4.631, 4.800, all P<0.05). Conclusion 2.0 mm clear corneal nonsuturing self-closing incision is superior to 3.5 mm self-closing sutureless transparent corneal incision in cataract surgery. It is worthy of clinical application.
