腹腔镜子宫肌瘤剔除术治疗子宫肌瘤的效果及对卵巢功能的影响

摘　　要：目的 研究子宫肌瘤采取腹腔镜子宫肌瘤剔除术治疗后产生的影响。方法 研究对象:62例子宫肌瘤患者。研究时间:2019年7月至2021年7月。研究分组方式:随机对照分组。研究分组类别:对照组传统开腹子宫肌瘤剔除手术,31例,观察组腹腔镜子宫肌瘤剔除术,31例。比较两组患者的治疗效果。结果 观察组医治后临床效果显著较优,观察组术中出血量、排气时间、下床活动时间、手术时间、住院时间均优于对照组,P <0.05。观察组术前、术后促卵泡激素、促黄体生成素无明显差异(P> 0.05),说明腹腔镜肌瘤剥除术后不影响卵巢功能。对照组术前、术后促卵泡激素、促黄体生成素有明显差异(P <0.05),说明传统开腹子宫肌瘤剔除手术对卵巢功能具有一定影响。观察组和对照组术前促卵泡激素、雌二醇、促黄体生成素均无差异(P> 0.05),术后促卵泡激素、雌二醇、促黄体生成素均具有明显差异(P <0.05)。观察组医治后QOL评分显著较高,观察组治疗后心理状况评分、社会功能评分、健康状况评分均高于对照组(P <0.05)。观察组治疗后并发症发生率和复发率显著低于对照组(P <0.05)。结论 子宫肌瘤采取腹腔镜子宫肌瘤剔除术对卵巢功能影响不大,并发症较少,对提高患者生活水平具有一定帮助,达到的医治效果较佳。Objective To study the effect of laparoscopic myomectomy on uterine leiomyoma.Methods Sixty-two patients with hysteromyoma were studied.Research time:July 2019 to July 2021.Study grouping method:randomized control group.Classification of study groups:31 patients in the control group received traditional open myomectomy,and 31 patients in the observation group received laparoscopic myomectomy.The therapeutic effects of the two groups were compared.Results The clinical effect of the observation group was significantly better after treatment.The intraoperative bleeding volume,the exhaust time,the activity time,the operation time,and the hospitalization time of the observation group was significantly better than that of control group,P<0.05.There was no significant difference between preoperative and postoperative follicle stimulating hormone and luteinizing hormone in the observation group(P>0.05),indicating that ovarian function was not affected after laparoscopic myomectomy.There were significant differences in follicle stimulating hormone and luteinizing hormone between the control group before and after surgery(P<0.05),indicating that traditional open myomectomy has some effect on ovarian function.There were no differences in follicle stimulating hormone,estradiol and luteinizing hormone between the observation and control groups before surgery(P>0.05),and there were significant differences in follicle stimulating hormone,estradiol and luteinizing hormone after surgery(P<0.05).The QOL score of the observation group after treatment was significantly higher.The psychological status score,social function score,and health status score of the observation group after treatment were higher than that of the control group,P<0.05.The complication rate and recurrence rate in the observation group after medical treatment was significantly lower than that in the control group(P<0.05).Conclusion Laparoscopic myomectomy for uterine leiomyoma has little effect on ovarian function and less complications.It is helpful to improve
