摘 要:目的分析泮托拉唑联合氨甲环酸治疗对急性上消化道出血患者免疫功能、氧化应激反应的影响。方法随机选择接诊的78例急性上消化道出血患者为研究对象,用数字表法分为对照组、观察组。两组均给予泮托拉唑药物治疗,观察组辅以氨甲环酸联合治疗。治疗14 d后,观察两组患者恢复状况,检测T淋巴细胞(CD3^(+)、CD4^(+)、CD8^(+))和免疫球蛋白G(immunoglobulin G,IgG)、免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白M(immunoglobulin M,IgM)、氧化应激[超氧化物歧化酶(superoxide dismutase,SOD)、丙二醛(malondialdehyde,MDA)、还原型谷胱甘肽过氧化物酶(glutathione peroxidase,GSH-Px)]水平,分析临床疗效。结果与对照组比较,治疗后观察组的止血有效率较高、再出血情况较低(P<0.05)。治疗后,与对照组比较,观察组T细胞亚群中CD3^(+)、CD4^(+)、IgG、IgA、IgM、SOD、GSH-Px水平较高,CD8^(+)、MDA水平较低(P<0.05)。治疗后,观察组病情复发率低于对照组(P<0.05)。结论泮托拉唑联合氨甲环酸可以提升急性上消化道出血患者的机体免疫功能,调节机体抗氧化平衡,促进患者恢复。Objective To investigate the effects of pantoprazole combined with tranexamic acid on immune function and oxidative stress in patients with acute upper gastrointestinal bleeding.Methods A total of 78 patients with acute upper gastrointestinal bleeding admitted in hospital were randomly selected as the study subjects,and divided into control group and experimental group by number table method.Both groups were given with pantoprazole drug therapy,and the experimental group was supplemented with tranexamic acid combination therapy.After 14 days of treatment,the recovery of the two groups was observed.T lymphocytes(CD3^(+),CD4^(+),CD8^(+)),Immunoglobulin G(IgG),Immunoglobulin A(IgA),Immunoglobulin M(Immunoglobulin M),and oxidative stress indicators,including superoxide dismutase(SOD),malondialdehyde(MDA),and glutathione peroxidase(GSH-px),were assessed.Results Compared with the control group,the effective rate of hemostasis was significantly higher in the experimental group,and the rebleeding significantly decreased after treatment(P<0.05).After treatment,compared with the control group,the levels of CD3^(+),CD4^(+),IgG,IgA,IgM,SOD,and GSH-Px in T cell subsets of the experimental group were significantly higher,while the levels of CD8^(+)and MDA were significantly lower(P<0.05).After treatment,the recurrence rate of the experimental group was significantly lower than that of the control group(P<0.05).Conclusion Pantoprazole combined with tranexamic acid can improve the immune function of patients with acute upper gastrointestinal bleeding,regulate the balance of antioxidant,and promote the recovery of patients.
