摘  要:目的观察盐酸羟考酮通过核转录因子κB(NF-κB)/诱导型一氧化氮合酶(iNOS)/一氧化氮(NO)通路在大鼠胃癌根治术中的超前镇痛效果及对应激反应的影响。方法取40只大鼠,其中30只采用N-甲基-N'-硝基-N-亚硝基胍(MNNG)诱导大鼠胃癌模型,剔除造模过程中死亡大鼠及术后组织鉴定非胃癌病变的大鼠共5只,25只大鼠随机分为模型组8只,前给药组8只,后给药组9只,剩余10只为对照组,各组均在水合氯醛腹腔麻醉下施行胃癌根治术。开腹前30 min,对照组、模型组腹腔注射生理盐水,前给药组腹腔注射盐酸羟考酮注射液;开腹后30 min,后给药组腹腔注射盐酸羟考酮注射液。比较各组大鼠术后24、48、72 h机械性缩足阈值(PWMT)、热缩足反射潜伏期(PWTL),血清一氧化氮(NO)、P物质(SP)浓度,脊髓组织谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)、丙二醛(MDA)水平,脊髓组织iNOS、NF-κB p65蛋白相对表达量。结果与对照组比较,模型组、前给药组、后给药组PWMT均增大,血清NO、SP浓度,脊髓组织MDA含量、iNOS、NF-κB p65蛋白相对表达量均升高,PWTL均延长,脊髓组织GSH-Px、SOD活力均下降(P<0.05);与模型组比较,前给药组、后给药组PWMT均减小,血清NO、SP浓度,脊髓组织MDA含量、iNOS、NF-κB p65蛋白相对表达量均降低,PWTL均缩短,脊髓组织GSH-Px、SOD活力均升高(P<0.05);前给药组PWMT小于后给药组,血清NO、SP浓度,脊髓组织MDA含量、iNOS、NF-κB p65蛋白相对表达量低于后给药组,PWTL长于后给药组,脊髓组织GSH-Px、SOD活力高于后给药组,差异有统计学意义(P<0.05)。结论盐酸羟考酮在大鼠胃癌根治术中具有超前镇痛作用,其作用机制可能是通过抑制NF-κB/iNOS信号通路发挥镇痛作用。Objective To observe the effect of oxycodone hydrochloride on the analgesic effect of nuclear transcription factorκB(NF-κB)/inducible nitric oxide synthase(iNOS)/nitric oxide(NO)pathway in the radical operation of gastric cancer in rats and the impact of stress response.Methods 40 rats were taken,30 of which were induced by N-methyl-N'-nitro-N-nitrosoguanidine(MNNG)model of gastric cancer in rats,and the dead rats and postoperative tissues during the modeling were excluded a total of 5 rats with non-gastric cancer lesions were identified.25 rats were randomly divided into 8 rats in models group,8 rats in pre-administration group,9 rats in post-administration group,and the remaining 10 rats in control group.All groups of rats underwent gastric cancer surgery under total anesthesia with hydrated chlorine.Thirty minutes before laparotomy,rats in the control group and model group were intraperitoneally injected with saline,and rats in the pre-administration group were intraperitoneally injected with oxycodone hydrochloride injection,30 minutes after laparotomy,rats in the post-administration group were intraperitoneally injected with oxycodine hydrochloride.24,48,and 72 hours after operation,comparison of paw withdrawal mechanical threshold(PWMT),paw withdrawal thermal latency(PWTL),serum nitric oxide(NO),substance P(SP)concentration,glutathione peroxidase(GSH-Px),superoxide dismutase(SOD),malondial dehyde(MDA)levels,iNOS,NF-κB p65 protein relative expression of spinal cord tissue in all groups of rats.Results Compared with the control group,the PWMT increased,the serum NO and SP concentrations,the MDA content of the spinal cord tissue,the relative expression of iNOS and NF-κB p65 protein all increased,PWTL were prolonged,and the GSH-Px and SOD activities of spinal cord tissue were decreased in model group,pre-ad-ministration group and post-administration group(P<0.05).Compared with the model group,the PWMT decreased,serum NO and SP concentrations,the MDA content of the spinal cord tissue,the relative expression
