摘  要:目的研究肺癌放疗CBCT与定位CT不同配准模式和不同配准区域的摆位误差。方法随机抽取25例肺癌放疗患者,在治疗前行千伏级锥形束CT(KV-CBCT)扫描,将重建获得的CBCT图像与定位CT图像分别使用基于整个CBCT图像区域的自动灰度配准(A方式)、自动骨配准(B方式)和基于计划靶区(PTV)外扩20 mm区域的自动灰度配准(C方式),得出不同配准方式在前后、头脚、左右方向及三个方向的旋转的误差值。采用配对t检验方法和χ2检验方法验证差异是否具有统计学意义,P<0.05为差异有统计学意义。结果A、B和C方式头脚(Y)平移和旋转误差有一定的统计学差异,但在前后方向平移和旋转以及左右方向平移和旋转误差A方式,B方式与C方式之间差异无统计学意义。误差绝对值≥3 mm的发生率在前后、左右方向A、B、C方式差异无统计学意义;在头脚方向,C方式显著高于A、B方式(P<0.05)。结论配准模式对肺癌图像引导放疗中CBCT图像配准有明显的影响。Objective To study the patient set-up errors of different matching modes between CBCT and CT images for lung cancer radiotherapy.Methods 25 patients with lung cancer radiotherapy were randomly selected and scanned with kilovoltage cone-beam CT(KV-CBCT)before treatment.The reconstructed CBCT and CT images were automatically matching using grey(mode A)and bone(mode B)methods on whole CBCT image area,and grey methods on the PTV+20 mm areas(mode C),respectively.The difference of set-up errors between different matching methods were compared.The paired t-test and Chi-square test were used to see the statistical significance,and p values of≤0.05 were considered statistically significant.Results The set-up errors of mode A,B and C on Y-axis had statistically significant differences(P<0.05),There had no statistically significant differences in the X-axis,and Z-axis between mode A,B and C.In the X-axis and Z-axis,the incidence of errors had no statistically significant differences,in Y-axis,mode C was significantly higher than mode A、B(P<0.05).Conclusion The matching area and methods affect the determination of set-up error in lung cancer radiotherapy patients.
