摘  要:目的探究吞噬细胞运动蛋白1和原钙黏附蛋白17启动子甲基化水平对胃癌的早期诊断价值。方法收集147例患者为研究对象,其中胃上皮内瘤变者49例,肠上皮化生者40例,胃癌患者58例,并且以142例体检健康者作为对照组。收集患者的一般资料,包括性别,年龄,等,收集患者的临床病理学资料,包括肿瘤大小,位置,分化程度,淋巴结转移,TNM分期等。应用甲基化检测试剂盒检测其甲基化水平,相关性检验采用Logistic回归分析。根据血清中甲基化的阳性率,检测其敏感性和特异性,绘制ROC曲线,并计算约登指数。结果肿瘤组高甲基化率为78.4%(1710个CpG位点),低甲基化率为21.6%(470个CpG位点)。与数据库对比,认为吞噬细胞运动蛋白1(Engulfment and cell motility 1,ELMO1)和原钙黏附蛋白17(Protocadherin 17,PCDH17)启动子甲基化水平变化最大。与对照组患者血浆中ELMO1和PCDH17的含量相比,胃癌组,上皮内瘤变组和肠化生组的血浆含量明显升高(P<0.05)。外周血中ELMO1和PCDH17启动子甲基化情况与肿瘤分化程度,淋巴结转移,浸润深度,CEA表达水平和Hp表达情况呈正相关(P<0.05)。外周血中ELMO1和PCDH17启动子甲基化水平与上皮内瘤变组和肠上皮化生组患者临床特征参数无相关性(P>0.05)。联合检测外周血中ELMO1和PCDH17甲基化后的约登指数和ROC曲线下面积也明显增加,高于任一单独指标或其他联合方式检测。结论ELMO1和PCDH17甲基化水平可以作为胃癌患者早期筛查的生物标志物。Objective To investigate the role of promoter methylation levels of Engulfment and Cell Motility 1 and Procalcitonin 17 in the early diagnosis of early gastric cancer.Methods 147 patients were collected as the research objects,including 49 cases of gastric intraepithelial neoplasia,40 cases of intestinal metaplasia and 58 cases of gastric cancer.142 healthy people were used as the control group.Collect the general baseline data of patients,including gender,age,etc.,and collect the clinicopathological data of patients,including tumor size,location,degree of differentiation,lymph node metastasis,TNM stage,etc.The methylation level was detected by methylation detection kit.Logistic regression analysis was used for correlation test.It was considered that P<0.05 had statistical difference.According to the positive rate of methylation in serum,the sensitivity and specificity were detected,the ROC curve was drawn,and the yoden index was calculated.Results In the tumor group,the hypermethylation rate was 78.4%(1710 CpG sites)and the hypomethylation rate was 21.6%(470 CpG sites).Compared with the database,it was considered that the methylation levels of Engulfment and cell motility 1(ELMO1)and Procalcitonin 17(PCDH17)promoters changed the most.The plasma levels of ELMO1 and PCDH17 in gastric cancer group,intraepithelial neoplasia group and intestinal metaplasia group were significantly higher than those in the control group(P<0.05).The methylation of ELMO1 and PCDH17 promoters in peripheral blood was positively correlated with the degree of tumor differentiation,lymph node metastasis,depth of invasion,CEA expression and HP expression(P<0.05).The methylation levels of ELMO1 and PCDH17 promoters in peripheral blood were not significantly different from those in patients with intraepithelial neoplasia and intestinal metaplasia(P>0.05).The Jordan index and the area under the ROC curve after methylation of ELMO1 and PCDH17 in peripheral blood were also significantly increased,which was higher than any single index or other com
