摘　　要：目的探讨以岗位胜任力为导向的评价体系在妇产科疑难病例多学科讨论诊治及病例演讲教学中应用的可行性。方法选取2018年7月至2021年6月在上海交通大学医学院附属瑞金医院妇产科住院医师规范化培训基地的住院医师41名,研究组20名参加妇产科疑难病例多学科讨论诊治及疑难病例演讲比赛,对照组21名参加传统教学病例讨论,建立以岗位胜任力为导向的阶段评价体系对两组进行评价,在研究组的疑难病例演讲比赛中建立现场评价体系。结果学习开始后,在同一时间段评价中,研究组的平均成绩明显高于对照组,差异有统计学意义(P<0.05)。研究组2020年演讲比赛的平均现场评价成绩为(93.30±2.72)分,明显高于2019年的(85.86±4.79)分,差异有统计学意义(P<0.05)。结论通过岗位胜任力为导向的评价体系在妇产科疑难病例多学科讨论诊治及病例演讲教学中的应用,加强了住院医师在疑难复杂病例中与患者沟通的技巧,提高了他们的多元思考、探索分析、团队协作以及演讲授课的能力,能大大提升住院医师的岗位胜任力。Objective To explore the feasibility of job competency oriented evaluation in multidisciplinary discussion and treatment of difficult cases and cases speech in the training for residents of Obstetrics and Gynecology.Methods A total of 41 r esidentswho participated in the s tandardized residency training of Obstetrics and Gynecology in Ruijin Hospital Affiliated to Shanghai Jiao Tong University,School of Medcine from July 2018 to June 2021 were selected in this study.20 residents in the study group participated in multidisciplinary discussion on diagnosis and treatment of difficult cases in gynecology and obstetrics and speech contest for difficult cases,while 21 residents in the control group participated in discussion of traditional teaching cases.A stage evaluation system guided by post competency was established to evaluate the two groups,and a field evaluation system was established in the speech contest for difficult cases in the research group.Results After learning,the average stage evaluation score of the study group was higher than that of the control group at the same evaluation time,with statistically significant difference(P<0.05).The score of scene evolution for cases speech of 2020 in the study group was(93.30±2.72),which was higher than that of 2019(85.86±4.79),with statistically significant difference(P<0.05).Conclusion The application of job competency oriented evaluation in the training of multidisciplinary discussion and treatment of difficult cases and cases speech can strengthen the communication skill,improve the ability of multiple thinking,exploration,analysis,teamwork,speech and teaching of the residents.Furthermore,it can greatly enhance the job competence of residents.
