摘 要:目的探讨急性髓系白血病(AML)患者维持化疗期外周血肿瘤坏死因子-α(TNF-α)、白介素-2(IL-2)、γ干扰素(IFN-γ)水平变化与其预后的关系。方法回顾性选取2019年5月至2020年5月在广东省肇庆市第一人民医院采取化疗治疗的54例AML患者,根据其临床化疗效果分为完全缓解组(n=29)和未完全缓解组(n=25)。收集患者维持化疗期的相关实验室数据,包括化疗前后TNF-α、IL-2、IFN-γ及白细胞计数(WBC)、血小板计数(PLT)、血红蛋白(Hb)水平,观察各组间患者指标水平差异,分析TNF-α、IL-2、IFN-γ与WBC、PLT及Hb水平变化的相关性;采用ROC曲线分析TNF-α、IL-2、IFN-γ与AML患者预后的关系。结果化疗后,完全缓解组患者的TNF-α水平低于未完全缓解组,IL-2、IFN-γ、PLT、Hb水平高于未完全缓解组,差异有统计学意义(P<0.05);两组WBC比较差异无统计学意义(P>0.05)。Pearson相关性分析结果显示,TNF-α与PLT、Hb呈负相关(r=-2.954、-3.142,P<0.05);IL-2与PLT、Hb呈正相关(r=3.214、3.326,P<0.05);IFN-γ与PLT、Hb呈正相关(r=3.359、3.478,P<0.05)。ROC曲线结果显示TNF-α、IL-2、IFN-γ及其联合检测均具有预测AML患者预后的预测价值(AUC=0.692、0.803、0.785、0.857,P<0.05)。结论TNF-α、IL-2、IFN-γ水平变化对AML患者预后具有预测价值,多指标联合检测价值更高。Objective To investigate the relationship between changes of tumor necrosis factor-α(TNF-α),interleukin-2(IL-2),and interferon-γ(IFN-γ)levels in the peripheral blood during maintenance chemotherapy and the prognosis of patients with acute myeloid leukemia(AML).Methods A total of 54 AML patients who underwent chemotherapy at the First People’s Hospital of Zhaoqing City from May 2019 to May 2020 were retrospectively selected and divided into the complete remission group(n=29)and the incomplete remission group(n=25)according to the clinical efficacy of chemotherapy.The relevant laboratory data during maintenance chemotherapy,including TNF-α,IL-2,IFN-γand white blood cell count(WBC),platelet count(PLT)and hemoglobin(Hb)levels before and after chemotherapy were collected.The differences in indicator levels of patients between the two groups were observed,and the correlations of TNF-α,IL-2 and IFN-γwith the changes of WBC,PLT and Hb levels were analyzed.In addition,the relationship between TNF-α,IL-2 and IFN-γlevels and the prognosis of AML patients was analyzed by using the Receiver Operating Characteristic(ROC)curve.Results After chemotherapy,the TNF-αlevels were lower and the IL-2,IFN-γ,PLT and Hb levels were higher in the complete remission group than those in the incomplete remission group,with statistically significant differences(P<0.05).There was no statistically significant difference between the two groups in the WBC(P>0.05).Pearson correlation analysis showed that the TNF-αlevel was negatively correlated with PLT and Hb levels(r=-2.954,-3.142,P<0.05),the IL-2 level was positively correlated with PLT and Hb levels(r=3.214,3.326,P<0.05),and the IFN-γlevel was positively correlated with PLT and Hb levels(r=3.359,3.478,P<0.05).The ROC curve showed that TNF-α,IL-2,and IFN-γalone,and the combination of them in detection all are of value in predicting the prognosis of AML patients(AUC=0.692,0.803,0.785,0.857,P<0.05).Conclusion Changes in TNF-α,IL-2,and IFN-γlevels can predict the prognosis of AM
