“补泻兼施”铺棉灸法在皮肤病中的应用

摘　　要：铺棉灸是由古代蛛网多层灸逐渐发展而来的外治方法,与传统艾灸相比有其独特的优势,尤其在治疗皮肤病方面具有丰富的经验和可靠的疗效。笔者根据临床跟诊和文献学习,系统整理并总结了铺棉灸规范操作流程及施术要点,并根据带状疱疹及带状疱疹后遗神经痛、神经性皮炎等不同皮肤病的病因病机及铺棉灸相应的应用特点,进一步分析铺棉灸针对不同病症的操作细节。目前对铺棉灸的补泻属性尚无定论,文献多强调其拔毒泻邪作用,本文通过整理文献结合及临床实践认为铺棉灸功效主要体现在补泻兼施,补可增益气血阴阳,温以通之,以通为补;泻可拔毒泻火,祛邪。Cotton spreading moxibustion is an external treatment method gradually developed from ancient spider web multi-layer moxibustion.Compared with moxibustion,it has unique clinical efficacy and is used for a variety of skin diseases.According to the etiology andpathogenesis of different diseases such as herpes zoster,postherpetic neuralgia and neurodermatitis,and the corresponding application characteristics of cotton spreading moxibustion,the operation characteristics of cotton spreading moxibustion for different symptoms were further analyzed.At present,there is no final conclusion on the reinforcing and reducing properties of cotton spreading moxibustion,and most literatures emphasize its function of detoxification and reducing pathogenic factors.Based on literature review and clinical practice,this paper holds that the efficacy of cotton spreading moxibustion is mainly reflected in reinforcing with reducing,which can increase Qi,Blood,Yin and Yang,warm and unblock,and remove pathogenic factors.
