摘　　要：目的探讨柴胡加龙骨牡蛎汤加减在卒中后抑郁中的应用效果。方法选取2018年4月至2019年12月荆州市第三人民医院收治的130例卒中后抑郁患者作为研究对象,采用随机数字表法分为试验组(65例)和对照组(65例)。对照组在常规干预基础上应用盐酸氟西汀分散片治疗,试验组在常规干预基础上应用柴胡加龙骨牡蛎汤加减治疗。比较两组的临床疗效及治疗前后的外周血同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)、5-羟色胺(5-HT)、脑源性神经营养因子(BDNF)和中性粒细胞与淋巴细胞比值(NLR)变化。结果试验组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,试验组血清Hcy、hs-CRP水平低于对照组,血浆5-HT、BDNF水平高于对照组,差异有统计学意义(P<0.05)。治疗后,试验组外周血NLR水平低于对照组,差异有统计学意义(P<0.05)。结论柴胡加龙骨牡蛎汤加减治疗卒中后抑郁疗效显著,可尽快纠正患者外周血Hcy、hs-CRP、5-HT、BDNF、NLR水平,缓解患者临床症状。Objective To investigate the effect of Modified Chaihu Longgu Muli Decoction on post-stroke depression.Methods A total of 130 patients with post-stroke depression admitted to the Jingzhou Third People's Hospital from April 2018 to December 2019 were selected as the research subjects,and they were divided into experimental group(65 cases)and control group(65 cases)according to the random number table method.The control group was treated with Fluoxetine Hydrochloride Capsules on the basis of routine treatment,and the experimental group was treated with Modified Chaihu Longgu Muli Decoction on the basis of routine treatment.The clinical efficacy and the changes of homocysteine(Hcy),high sensitive C-reaction protein(hs-CRP),5-hydroxytryptamine(5-HT),brain derived neurotrophic factor(BDNF)and neutrophil to lymphocyte ratio(NLR)before and after treatment were compared between the two groups.Results The total effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of serum Hcy and hs-CRP in the experimental group were lower than those in the control group,and the levels of plasma 5-HT and BDNF in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the level of NLR in peripheral blood of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Modified Chaihu Longgu Muli Decoction is effective in the treatment of post-stroke depression,it can correct the levels of Hcy,hs-CRP,5-HT,BDNF and NLR in the patients'peripheral blood as soon as possible,and relieve the patients'clinical symptoms.
