摘 要:目的 探讨特异性因子沉默信息调节因子2相关酶1(SIRT1)、过氧化物酶体增殖活化受体γ辅助活化因子1α(PGC-1α)在原发性开角型青光眼(POAG)患者中表达的意义,并观察其与小梁网氧化应激损伤的相关性。方法 回顾性分析2019年6月至2021年6月西安市第一医院收治的65例POAG患者(65眼)的临床资料,为POAG组,均行手术切除术获得小梁网组织样本;另取40例非眼部疾病者小梁网供体样本为对照组。苏木精-伊红(HE)染色法观察2组小梁网组织病理学变化;检测并对比POAG组与对照组小梁网组织SIRT1、PGC-1α mRNA和蛋白表达水平;根据POAG患者视野平均缺损程度将其分为早中期组和晚期组,对比2组小梁网组织SIRT1、PGC-1α mRNA和蛋白表达水平及氧化应激指标过氧化物酶(POD)、超氧化物歧化酶(SOD)水平。采用Pearson相关性分析法分析POAG患者SIRT1、PGC-1α mRNA和蛋白表达水平与氧化应激指标的相关性。结果 小梁网组织病理观察,对照组小梁薄板细长束状排列,小梁细胞束排列规则,小梁细胞形态正常;POAG组小梁束增厚,小梁网结构紊乱、板层交错,小梁细胞呈现数量减少,核固缩现象。POAG组小梁网组织中SIRT1、PGC-1α mRNA和蛋白相对表达量均低于对照组(P<0.05);POAG晚期组小梁网组织中SIRT1、PGC-1α mRNA和蛋白相对表达量均低于POAG早中期组(P<0.05);POAG晚期组小梁网组织中POD水平低于POAG早中期组(P<0.05),SOD水平高于POAG早中期组(P<0.05);Pearson相关分析显示,POAG患者小梁网组织SIRT1、PGC-1α mRNA和蛋白表达水平均与POD水平呈负相关(P<0.05),与SOD水平呈正相关(P<0.05)。结论特异性因子SIRT1、PGC-1α在POAG患者中异常低表达,且与小梁网组织氧化应激反应关系密切。Objective To investigate expression of the specific factor of silent information regulator factor 2 releated enzyme 1(SIRT1) and peroxisome proliferator-activated receptor γ-assisted activator factor 1α(PGC-1α) in patients with primary open angle glaucoma(POAG), and to observe its relationship with oxidative stress injury of trabecu-lar meshwork. Methods The clinical data of 65 POAG patients(65 eyes) admitted to Xi′an NO.1 Hospital from June2019 to June 2021 were retrospectively analyzed. All patients in POAG group underwent surgical resection to obtain trabecular meshwork tissue samples. Another 40 trabecular meshwork donors from non-ocular diseases were selected as control group. Histopathological changes of trabecular meshwork in two groups observed by hematoxylin-eosin staining(HE). The SIRT1 and PGC-1α expressions in trabecular meshwork tissue between POAG group and control group were detected and compared. According to the average degree of visual field defect in POAG patients, they were divided into early-middle stage group and late-stage group.The mRNA and protein expressions of SIRT1, PGC-1α and oxidative stress index,including peroxisome(POD)and superoxide dismutase(SOD)in trabecular meshwork tissue of the two groups were compared.Pearson correlation analysis was used to analyze the correlation between the expression of SIRT1,PGC-1α and oxidative stress in POAG patients. Results Histopathological observation of trabecular meshwork showed that the trabecular meshwork in the control group was arranged in slender bundles with a regular arrangement of trabecular cell bundles and a normal shape of trabecular cells. In POAG group, the trabecular meshwork became thick-er, the structure of trabecular meshwork was disordered and the lamellae were staggered, the number of trabecular cells decreased and the nucleus contracted. The relative expressions of SIRT1, PGC-1α mRNA and protein in the trabecular meshwork of the POAG group were lower than those of the control group(P<0.05). The relative expressions
