青年性胶质母细胞瘤MRI表现和免疫组化相关性研究

摘　　要：目的探讨青年性胶质母细胞瘤MRI表现与免疫组化之间关系,提高术前诊断水平。方法选取观察3项MRI表现指标:增强CE/T_(2)比值、是否多灶性生长、是否出现坏死区,统计3项指标分别与P53、Ki-67和IDH1之间关系,所有的结果使用统计软件SPSS 26.0进行分析。结果P53低表达18例(56.3%),高表达14例(43.7%)。9例(28.1%)CE/T_(2)<1,23例(71.9%)CE/T_(2)≥1,差异有统计学意义(P<0.05);Ki-67低表达24例(75.0%),高表达8例(25.0%)。肿瘤多灶性生长7例(21.9%),局限性生长25例(78.1%),Ki-67水平与肿瘤是否多灶性生长差异有统计学意义(P<0.05);IDH1低表达17例(53.1%),高表达15例(46.9%)。肿瘤内无坏死区11例(34.4%),出现坏死区21例(65.6%),IDH1表达水平与肿瘤是否出现坏死区差异有统计学意义(P<0.05)。结论增强CE/T_(2)比值、肿瘤是否多灶性、肿瘤内是否出现坏死区等3项MRI影像学特征与青年性胶质母细胞瘤的免疫组化结果P53、Ki-67和IDH1相关。术前MRI检查可获取青年性胶质母细胞瘤的肿瘤分子病理学特征,具有一定的临床诊断指导价值。Objective To investigate the association between the magnetic resonance imaging(MRI)findings and immunohistochemical profiles of glioblastoma in young patients,in order to improve the capability of accurate diagnosis.Methods 32 cases of glioblastoma were retrospectively analyzed.Three imaging features such as contrast-enhanced CE/T_(2)ratio,multifocality and tumor necrosis were selected to identify the relationship with P53,Ki-67 and IDH1 expression,using the software of SPSS 26.0 and Chi-square test.Results High expression of P53 indicated a high frequency of contrast-enhanced CE/T_(2)ratio(P<0.05).High expression of Ki-67 was correlated with multifocality(P<0.05).Low expression of IDH1 was correlated with tumor necrosis(P<0.05).Conclusion Some MRI features such as contrast-enhanced CE/T_(2)ratio,multifocality and tumor necrosis are correlated with P53,Ki-67 and IDH1 of glioblastoma in young patients.
