摘  要:目的探讨3D TOF MRA在永存颈动脉-椎基底动脉吻合诊断中的应用。方法选取我院行头颈部MRA检查的129例永存颈动脉-椎基底动脉吻合患者,分析其起源、路径、椎基底动脉发育状况及其他合并变异。结果129例永存颈动脉-椎基底动脉吻合中:PTA 113例,左侧58例,右侧55例。Weon分型Ⅰ型69例,Ⅱ型14例,Ⅲ型12例,Ⅳ型11例,Ⅴ型7例,均为Ⅴa型。102例合并椎动脉发育不良或闭塞。PHA 10例,左侧3例,右侧7例,均合并对侧椎动脉发育不良。PIA 6例,左侧5例,Ⅰ型1例,Ⅱ型4例;双侧1例,为Ⅱ型;4例合并对侧椎动脉发育不良。结论3D TOF MRA能明确诊断永存颈动脉-椎基底动脉吻合变异、分型,为临床手术提供可靠的形态学依据。Objective To explore the application of 3D TOF MRA in the diagnosis of persistent carotid-vertebrobasilar artery anastomoses.Methods 129 cases of permanent carotid-vertebrobasilar artery anastomoses underwent 3D TOF MRA in our hospital were collected.The origin,route,development of vertebrobasilar artery and other concomitant variations were analyzed.Results In 129 cases of persistent carotid-vertebrobasilar artery anastomoses.113 cases were PTA,58 cases were located on the left and 55 cases on the right.According to Weon’s classification,there were 69 cases of typeⅠ,14 cases of typeⅡ,12 cases of typeⅢ,11 cases of typeⅣand 7 cases of typeⅤa.102 cases were complicated with vertebral artery dysplasia or occlusion.10 cases were PHA,3 cases were located on the left and 7 cases on the right,all of them were complicated with contralateral vertebral artery dysplasia.6 cases were PIA,5 cases were located on the left,1 case was typeⅠ,4 cases were typeⅡ.1 case was bilateral,typeⅡ.4 cases were complicated with contralateral vertebral artery dysplasia.Conclusion 3D TOF MRA can accurately diagnose the variation and classification of persistent carotid-vertebrobasilar artery anastomoses,and provide reliable morphological basis for clinical operation.
