颈脑一体化血管超声检查对缺血性脑血管病的诊断价值

摘　　要：目的探讨缺血性脑血管病中颈脑一体化血管超声检查的诊断价值。方法选取我院收治的100例缺血性脑血管病患者,所有患者均接受颈动脉超声、经颅彩色多普勒超声(TCCD)及数字减影血管造影检查(DSA),以DSA为“金标准”,分析患者血管病变情况。结果100例存在颅内、外血管病变的患者中,颈动脉超声检出异常血管共计291条;经颅多普勒超声检出颅内异常血管共计246条;颈脑一体化血管超声检出异常血管共计673条;DSA检出颈部血管异常共计410条,检出颅内异常血管共计315条,异常血管共计725条。在血管狭窄检出率及对缺血性脑血管检测准确性方面,颈脑一体化血管超声检查与DSA比较差异无统计学意义(P>0.05)。结论颅内外血管病变诊断中颈脑一体化血管超声检查有利于缺血性脑病患者早诊断、早治疗,其诊断准确率仅次于DSA,但无法完全替代DSA,临床医师在诊断时需综合临床症状合理选用诊断方式,避免漏诊、误诊,及时对患者采取对症治疗。Objective To discuss the diagnostic value of integrated carotid and cerebral vascular ultrasound in ischemic cerebrovascular disease.Methods 100 patients with ischemic cerebrovascular disease treated in our hospital were selected in this work.All patients received carotid ultrasound,transcranial color coded Doppler,and digital subtraction angiography.The results of digital subtraction angiography were taken as the"gold standard"to analyze the vascular disease of patients.Results Among 100 patients with intracranial and external vascular diseases,291 abnormal vessels were detected by carotid ultrasound;A total of 246 abnormal intracranial vessels were detected by transcranial Doppler ultrasound;A total of 673 abnormal blood vessels were detected by carotid cerebral integrated vessel ultrasound;DSA detected 410 abnormal cervical vessels,315 abnormal intracranial vessels and 725 abnormal vessels.In terms of the detection rate of vascular stenosis and the accuracy of ischemic cerebrovascular detection,there was no significant difference between carotid cerebral integrated vascular ultrasound and DSA(P>0.05).Conclusion The diagnosis of extracranial and extracranial vascular diseases,transcranial integrated vascular ultrasound can provide more reference for clinicians,which is conducive to the early diagnosis and treatment of patients with ischemic encephalopathy,but,it can not completely replace the digital subtraction angiography of the head.Clinicians should combine the patient's physical signs,reasonably select the diagnosis method according to the clinical symptoms,avoid missed diagnosis and misdiagnosis,and timely take symptomatic treatment for patients.
