摘  要:目的探讨CT增强静脉期图像组学特征,对比常规影像特征在鉴别原发性和转移性卵巢癌中的应用价值。方法选取经病理证实的99例卵巢浆液性囊腺癌和48例卵巢转移癌,回顾性分析其年龄及常规影像特征,包括静脉期CT值、大小、单双侧、囊实性、分隔厚度及是否伴发腹水。应用AK软件(Analysis-Kit,GE healthcare)提取肿瘤静脉期的组学特征,并通过IPMs(IPM Statistics,GE healthcare)软件进行特征筛选及构建模型。绘制受试者工作特征曲线(ROC)评价其鉴别诊断效能。结果常规影像特征中,两组患者的年龄及肿块的分隔厚度差异有统计学意义(P<0.05),卵巢转移癌患者的年龄显著低于浆液性囊腺癌患者,浆液性囊腺癌更容易出现较厚的分隔。ROC分析常规影像特征的AUC在训练集和验证集中分别为0.700、0.671。训练集中浆液性囊腺癌和卵巢转移癌的Radscore值分别为-1.594±1.165 vs.-0.812±0.965(P=0.001);验证集中Radscore值分别为-1.437±1.140 vs.-0.636±1.087(P=0.029)。ROC分析组学特征在训练集和验证集中的AUC分别为0.834、0.682,均高于常规影像学特征。结论基于CT增强静脉期图像的组学分析在鉴别原发性卵巢癌和转移性卵巢癌中具有重要的临床价值。Objective To investigate the value of radiomics features of CT enhanced venous phase images compared with conventional imaging features in the differentiation of primary ovarian cancer and metastatic ovarian cancer.Methods 99 cases of ovarian serous cystadenocarcinoma and 48 cases of ovarian metastatic cancer confirmed by pathology were selected and their age and imaging parameters were analyzed retrospectively,including venous CT value,size,laterality,cystic and solid,separation and ascites.The solid components of tumor lesions were manually drawn on all slices of venous contrast-enhanced images and CT radiomics parameters were extracted in AK software(Analysis-Kit,GE healthcare).IPMS(IPM statistics,GE Healthcare)software was used to select features and to build models.The receiver operating characteristic(ROC)was drawn to evaluate the efficacy of differential diagnosis.Results Age and mass separation thickness existed statistical differences between the two groups.The age of patients with ovarian metastatic cancer was significantly lower than that of serous cystadenocarcinoma,and serous cystadenocarcinoma was more prone to formthick separation.ROC analysis showed that the AUC of conventional image features in training set and validation set were 0.700 and 0.671 respectively.10 radiomics parameters of venous phase were selected and their Radscore values were calculated.In training set,the Radscore value of serous cystadenocarcinoma and metastatic cancer were-1.594±1.165 vs.-0.8116±0.965(P=0.001),respectively;In validation set,the Radscore values of serous cystadenocarcinoma and metastatic cancer were-1.437±1.140 vs.-0.636±1.087(P=0.029),respectively.ROC analysis showed that the AUC of radiomics features in training set and validation set were 0.834 and 0.682 respectively,which was higher than that of traditional imaging features(training set 0.700:verification set 0.671).Conclusion Radiomics analysis based on CT enhanced venous images has important clinical value in differential diagnosis of primary ovarian
