TCT、HPV联合阴道镜下定位活检在宫颈癌筛查中的应用研究

摘　　要：目的分析宫颈液基细胞检查(TCT)、人乳头瘤病毒(HPV)联合阴道镜下定位活检在宫颈癌筛查中的应用效果。方法选取72例进行筛查的疑似宫颈癌或宫颈高度病变患者为研究对象,所有患者经HPV或TCT筛查均提示有异常,后经阴道镜镜下定位活检病理提示为宫颈高度病变或宫颈癌。追踪分析所有患者临床资料,观察TCT、HPV联合阴道镜下定位活检的诊断方式用于宫颈癌筛查中的诊断效能。结果本组所有患者经临床手术治疗后病理诊断发现均为阳性,阳性符合率100%;单一TCT诊断符合率为84.7%(61/72),单一HPV诊断符合率为87.5%(61/72),两种检查方法比较差异无统计学意义(P>0.05);TCT、HPV联合阴道镜应用诊断准确率均高于两种单一诊断,差异有统计学意义(P<0.05)。结论临床上对宫颈癌患者实施筛查过程中,综合应用TCT、HPV联合阴道镜下定位活检,诊断结果的准确率更高,能够为宫颈癌筛查以及临床治疗提供可靠的指导依据,建议推广应用。Objective To analyze the application effect of cervical cell-based examination(TCT),human papillomavirus(HPV)combined with colposcopic localization biopsy in cervical cancer screening.Methods 72 patients with suspected cervical cancer or high cervical lesions were selected as subjects.All patients showed abnormalities after HPV or TCT screening,and then showed high cervical lesions or cervical cancer after location biopsy under colposcopy.Clinical data of all patients were tracked and analyzed to observe the diagnostic efficacy of TCT and HPV combined colposcopic localization biopsy in cervical cancer screening.Results All patients in this group were found to be positive in pathological diagnosis after clinical surgery,and the positive coincidence rate was 100%.The coincidence rate of single TCT diagnosis was 84.7%(61/72),and the coincidence rate of single HPV diagnosis was 87.5%(61/72),There was no significant difference between the two methods(P>0.05).The diagnostic accuracy of TCT and HPV combined colposcopy was higher than that of two single diagnoses,and the difference was statistically significant(P<0.05).Conclusion TCT,HPV combined with colposcopy guided biopsy can provide more accurate diagnostic results during the clinical screening of cervical cancer patients,which can provide reliable guidance for cervical cancer screening and clinical treatment.It is recommended to promote the application.
