摘　　要：目的:观察半胱氨酸蛋白酶抑制剂C(Cys C)在胃癌患者血清中的水平及组织中的表达,进而了解其在胃癌中的诊断价值及对胃癌侵袭与转移的影响。方法:酶联免疫吸附法(ELISA)检测40例正常患者(正常组)、40例早期胃癌患者(早期胃癌组)、40例进展期胃癌患者(进展胃癌组)血清中Cys C及癌胚抗原(CEA)的水平并进行比较,绘制ROC曲线评估CysC与CEA在胃癌诊断中的价值;免疫组化检测正常患者胃组织、早期癌和进展期癌癌组织中以及进展期癌癌旁组织中Cys C的表达情况。结果:血清中Cys C和CEA在正常组、早期胃癌组、进展胃癌组三组中的水平呈增高趋势,且各组之间的表达差异有统计学意义(P<0.05),且Cys C和CEA的ROC曲线下面积相近;组织中Cys C在正常组、早期胃癌组、进展胃癌组三组中的表达逐渐增高,进展期癌组织较癌旁组织中的表达高,各组之间的表达差异均有统计学意义(P<0.05)。结论:人半胱氨酸蛋白酶抑制剂C在胃癌诊断中有一定的价值,可能参与了胃癌的侵袭与转移。Objective To observe the level of cysteine protease inhibitor C(Cys C) in serum and the expression of Cys C in tissues of patients with gastric cancer,so as to understand its diagnostic value in gastric cancer and its influence on invasion and metastasis of gastric cancer.Method The levels of Cys C and carcinoembryonic antigen(CEA) in serum of 40 normal patients(normal group),40 patients with early gastric cancer(early gastric cancer group) and 40 patients with advanced gastric cancer(advanced gastric cancer group) were detected and compared by enzyme-linked immunosorbent assay(ELISA),ROC curve was drawed to evaluate the value of CysC and CEA in the diagnosis of gastric cancer;The expression of Cys C in normal gastric tissues,early cancer tissues,advanced cancer tissues and tissues adjacent to advanced cancer tissues was detected by immunohistochemistry.Results The levels of Cys C and CEA in serum in normal group,early gastric cancer group,and advanced gastric cancer group showed an increasing trend,and the expression of Cys C and CEA in each group was statistically significant,and the area under the ROC curve of Cys C and CEA was similar;The expression of Cys C in tissues of normal group,early gastric cancer group and advanced gastric cancer group increased gradually;The expression of Cys C in advanced cancer tissues was higher than that in adjacent cancer tissues,and the difference between groups was statistically significant(P<0.05).Conclusion Human cysteine protease inhibitor C has a certain value in the diagnosis of gastric cancer,and may be involved in the invasion and metastasis of gastric cancer.
