摘　　要：目的:探讨老年急性髓系白血病(AML)患者采用MA与CAG两种不同方案诱导治疗的可行性和安全性。方法:回顾性分析2012年6月~2018年6月我院血液科收治的78例老年AML患者的临床资料,其中42例采用MA方案、36例采用小剂量CAG方案诱导化疗。比较两组患者化疗后半年的治疗效果、不良反应发生率和治疗前后的生活质量。结果:在临床疗效方面,MA组和小剂量CAG组总有效缓解率无明显差异(P>0.05)。在不良反应发生率方面,CAG组骨髓抑制和肺部感染发生率明显低于MA组(均P<0.05)。治疗后两组患者生理功能评分、精神健康和情感职能评分较治疗前均有所提高,但组间差异无统计学意义(均P>0.05)。结论:CAG方案与MA方案治疗老年AML的疗效相当,但CAG在化疗期间的不良反应发生率更低。Objective:To investigate the feasibility and safety of MA and CAG in elderly patients with acute myeloid leukemia(AML).Methods:The clinical data of 78 elderly AML patients in our hospital from June 2012 to June 2018 were retrospectively analyzed.There were 42 cases treated with MA regimen,and 36 cases treated with low-dose CAG regimen.The clinical efficacy,incidence of adverse reaction,and quality of life before and after chemotherapy were compared between the two groups.Results:In terms of clinical efficacy,there was no significant difference in the total effective remission rate between MA group and CAG group(P>0.05).Compared to the MA group,the incidence of bone marrow suppression and pulmonary infection were significantly reduced in the low-dose CAG group(both P<0.05).The scores of physiological function,mental health and emotional function were improved when compared with those before treatment,while there were no statistical differences between the two groups(all P>0.05).Conclusion:CAG regimen could be comparable to MA regimen in the treatment of elderly AML,while CAG has a lower incidence of adverse reaction during chemotherapy.
