摘 要:目的:了解2017—2021南宁市肺结核健康管理效果,探讨规则服药率的空间分布规律,为南宁市肺结核健康管理提供新思路。方法:选择南宁市12个县(市)区共126个乡镇/街道为研究区域,利用广西基本公共卫生服务项目管理信息网络直报系统中2017—2021年肺结核患者规则服药数据计算规则服药率。采用ArcGIS 10.3软件对肺结核规则服药率作空间自相关分析。结果:2017—2021年南宁市总肺结核规则服药率为95.65%,空间自相关分析结果显示,2017—2021年南宁市肺结核规则服药率具有空间聚集性(Moran’s I=0.022402,Z=2.005071,P<0.05);7个“高—高”聚集区分布在横州市那阳镇、马岭镇、南乡镇、百合镇、横州镇、马山乡、莲塘镇;6个“低—低”聚集区包括马山县周鹿镇、里当瑶族乡、古寨瑶族乡、乔利乡、金钗镇和武鸣区府城镇。结论:2017—2021年南宁市肺结核规则服药率已达到国家基本公共卫生服务项目要求的指标值,同时规则服药率也呈现地区差异性,建议今后在管理患者服药工作中针对不同需求的人群推广使用智能工具,以提高规则服药率。Objective:To figure out the effects of tuberculosis health management in Nanning from 2017 to 2021 and explore the spatial distribution of regular medication rate so as to provide new ideas for tuberculosis health management in Nanning city.Methods:A total of 126 townships/streets in 12 counties(cities)and districts of Nanning city were selected as the study area and the regular medication rate of tuberculosis patients was calculated by using the 2017-2021 regular medication data from the direct reporting system of Guangxi basic public health service project management information network.ArcGIS 10.3 software was used to make spatial autocorrelation analysis on the regular medication rate of pulmonary tuberculosis.Results:The total regular medication rate of tuberculosis in Nanning city during 2017-2021 was 95.65%.The results of spatial autocorrelation analysis showed that the regular medication rate of tuberculosis in Nanning city during 2017-2021 had spatial aggregation(Moran's I=0.022402,Z=2.005071,P<0.05).Seven"high-high"gathering areas were distributed in Nayang Town,Maling Town,Nanxiang Town,Baihe Town,Hengzhou Town,Mashan Township and Liantang Town of Hengzhou City;the six"low-low"gathering areas included Zhoulu Town in Mashan County,Lidang Township of Yao Ethnic Group,Guzhai Township of Yao Ethnic Group,Qiaoli Township,Jinchai Town and Fucheng town in Wuming District.Conclusion:From 2017 to 2021,the regular medication rate of tuberculosis in Nanning City has reached the index value required by the national basic public health service project,and the regular medication rate also presents regional differences.It is suggested to promote the use of intelligent tools in the management of patients'medication for people with different needs in order to improve the regular medication rate.
