摘　　要：目的探讨超声引导下经皮微波消融术(MWA)治疗子宫肌瘤(UM)患者对肌瘤残留率及血清结缔组织生长因子(CTGF)、恶性肿瘤相关物质群(TSGF)水平的影响。方法收集2020年1月至2022年1月太仓市妇幼保健计划生育服务中心收治的80例UM患者资料,根据手术方法的差异分为两组,观察组(43例)行超声引导下MWA,对照组(37例)行腹腔镜子宫肌瘤剔除术(LM),比较两组患者CTGF、TSGF水平及肌瘤残留率、复发率,并统计术后并发症发生情况。结果术后,两组患者CTGF水平均升高,TSGF水平降低,观察组CTGF、TSGF水平低于对照组(P<0.05);观察组患者术后3 d肌瘤残留率及术后3个月复发率均低于对照组(P<0.05);观察组术后并发症总发生率为6.98%,低于对照组的24.32%(P<0.05)。结论超声引导下MWA能够改善血清CTGF、TSGF水平,降低UM患者肌瘤残留率及复发率,安全性较高。Objective To investigate the effect of ultrasound-guided percutaneous microwave ablation(MWA)on the residual rate of fibroids,the levels of serum connective tissue growth factor(CTGF)and malignant tumor supplied group of factors(TSGF)in patients with uterus myoma(UM).Methods A total of 80 UM patients admitted to Taicang Maternal and Child Health and Family Planning Service Center from January 2020 to January 2022 were collected,based on the differences in surgical methods,they were divided into two groups.The observation group(43 cases)underwent ultrasound-guided percutaneous MWA,and the control group(37 cases)underwent laparoscopic myomectomy(LM).The levels of CTGF and TSGF and the residual rate,recurrence rate of fibroids were compared between the two groups,and the incidence of postoperative complications was counted.Results After operation,the levels of CTGF in the two groups increased,and the levels of TSGF decreased.The levels of CTGF and TSGF in the observation group were lower than those in the control group(P<0.05).The residual rate,recurrence rate of fibroids at 3 days after operation and the recurrence rate at 3 months after operation in the observat ion group were lower than those in the control group(P<0.05).T he total incidence of pos toperative complications in the obs ervation group was 6.98%,which was lower than 24.32%in the control group(P<0.05).Conclusion Ultrasound-guided percutaneous MWA can improve serum CTGF and TSGF levels,reduce the residual rate and recurrence rate o f fibroids in patients with UM,and has high safety.
