rhEGF凝胶对口腔颌面部外伤创面清创术后局部微循环的影响及美容修复效果观察

摘　　要：目的:探讨重组人表皮生长因子(Recombinant human epidermal growth factor,rhEGF)凝胶对口腔颌面部外伤创面清创术后局部微循环的影响,并观察其修复效果。方法:对83例口腔颌面部外伤患者的临床资料进行回顾分析,均实施清创术,其中40例术后仅予以基础对症治疗(A组);43例术后予以基础对症治疗+rhEGF凝胶(B组)。对比创面结痂时间、岛状上皮形成时间、创面愈合时间,术后即刻、术后3 d和术后7 d局部微循环指标变化及两组术后3个月修复效果;对比两组并发症发生率,包括创面渗液、创面感染、瘢痕形成等。结果:B组创面结痂时间、岛状上皮形成时间、创面愈合时间均短于A组(P<0.05);血清钙、磷水平组间、时间及交互作用对比差异均无统计学意义(P>0.05),血液灌流量(PU值)组间、时间及交互作用对比差异均有统计学意义(P<0.05),两组术后3 d、术后7 d PU值均高于术后即刻,术后7 d均高于术后3 d,且B组术后3 d、术后7 d PU值均高于A组,差异均有统计学意义(P<0.05);两组术后3个月美容修复效果对比差异有统计学意义(P<0.05),且B组优良率高于A组(P<0.05);B组并发症发生率低于A组(P<0.05)。结论:rhEGF凝胶可改善口腔颌面部外伤创面清创术后局部微循环,加快创面结痂和愈合,提升美容修复效果,减少并发症的发生。Objective To investigate the effect of recombinant human epidermal growth factor(rhEGF) gel on local microcirculation after debridement of oral and maxillofacial wounds and observe its cosmetic role. Methods The clinical data of 83 patients with oral and maxillofacial trauma were reviewed, and among them, 40 cases were only treated with basic symptomatic treatment, which was recorded as group A. In addition, 43 patients were given symptomatic treatment of +rhEGF gel after operation, as group B. The time of scab formation, island epithelium formation and wound healing were compared.The changes of local microcirculation indexes were compared immediately, 3 days and 7 days after operation. The cosmetic effect after 3 months of operation were compared between the two groups. The complications of the two groups were compared,including wound exudation, wound infection and scar formation. Results The time of scab formation, island epithelium formation and wound healing in group B were shorter than those in group A(P<0.05). There was no significant difference in serum calcium and phosphorus levels among groups, moments and interaction(P>0.05), and there were significant differences in blood perfusion volume(PU values) among groups, moments and interaction(P<0.05). The Pu values of the two groups after 3 d and 7 d of operation were higher than those of the immediate postoperative days(P<0.05), which after 7d of operation were higher than those after 3 d of operation(P<0.05). The values of PU in group B after 3 d and 7 d of operation were higher than those in group A(P<0.05). There was a significant difference in the distribution of cosmetic effect between the two groups(P<0.05), and the excellent rate of group B was higher than that of group A(P<0.05). The incidence of complications in group B was lower than that in group A(P<0.05). Conclusion rhEGF gel can improve the local microcirculation after oral and maxillofacial wound debridement, accelerate the scab and healing, enhance the cosmetic effect,and reduce complication
