前列腺癌患者错配修复基因表达、微卫星不稳定性与临床病理特征的相关性研究

摘  要:目的:探讨前列腺癌患者DNA错配修复(MMR)基因表达、微卫星不稳定性(MSI)与临床病理特征的相关性。方法:选取2020年2月-2022年10月梅州市中医医院泌尿外科收治的前列腺癌患者82例为研究对象。收集患者病理蜡块,用免疫组化法检测标本中MLH1、PMS2、MSH2、MSH6蛋白表达情况,根据四种蛋白的表达情况识别前列腺癌中MSI状态分为错配修复功能缺陷(dMMR/MSI-H)组32例和非dMMR/MSI-H组50例。比较免疫组化法检测结果,分析dMMR/MSI-H型前列腺癌与组织学分级、肿瘤临床分期、预后及复发情况。结果:前列腺癌患者MLH1、PMS2、MSH2、MSH6经免疫组化法检测阳性率依次为74.4%、79.3%、82.9%、85.4%;两组患者组织学分级与肿瘤临床分期比较,差异有统计学意义(P<0.05),MMR/MSI患者组织学分级与肿瘤临床分期更高;dMMR/MSI-H组中dMMR/MSI-H表达情况与组织学分级、肿瘤临床分期呈负相关,差异有统计学意义(P<0.05);两组患者预后及复发情况比较,差异无统计学意义(P>0.05)。结论:前列腺癌患者MMR蛋白缺失与MSI可能和组织学分级、肿瘤临床分期相关,dMMR/MSI-H型患者可能具有预后更好,复发风险更低的特征。Objective:To investigate the correlation between DNA mismatch repair(MMR)gene expression,microsatellite instability(MSI)and clinicopathological features in patients with prostate cancer.Methods:A total of 82 patients with prostate cancer admitted to the Department of urology of Meizhou Traditional Chinese Medicine Hospital from February 2020 to October 2022 were selected as the study objects.The pathological wax blocks of patients were collected and the protein expressions of MLH1,PMS2,MSH2 and MSH6 in the specimens were detected by immunohistochemistry.According to the expression of four proteins,MSI status in prostate cancer was identified and divided into dMMR/MSI-H group(32 cases)and non-DMMR/MSI-H group(50 cases).The results of immunohistochemistry were compared to analyze the histological grade,clinical stage,prognosis and recurrence of dMMR/MSI-H prostate cancer.Results:The positive rates of MLH1,PMS2,MSH2 and MSH6 in prostate cancer patients were 74.4%,79.3%,82.9%and 85.4%,respectively.There was significant difference in histological grade and clinical stage between the two groups(P<0.05).MMR/MSI patients had higher histological grade and clinical tumor stage.The expression of dMMR/MSI-H in dMMR/MSI-H group was negatively correlated with histological grade and clinical stage of tumor,and the difference was statistically significant(P<0.05).There was no significant difference in prognosis and recurrence between the two groups(P>0.05).Conclusion:MMR protein deficiency in prostate cancer patients may be related to MSI,histological grade and clinical stage of tumor,and dMMR/MSI-H patients may have better prognosis and lower risk of recurrence.
