摘 要:目的 探讨E-Max玻璃陶瓷高嵌体对行后牙冠修复患者牙周指数及远期后牙折裂率的影响。方法 84例(84颗患牙)行后牙冠修复患者,依据随机数字表法分为对照组和观察组,各42例(42颗患牙)。对照组予以常规钴铬烤瓷冠修复,观察组予以E-Max玻璃陶瓷高嵌体修复。比较两组患者修复效果、修复成功率、修复前后的牙龈指数(GI)、龈沟出血指数(SBI)、牙周袋深度(PD)、附着丧失(AL)水平、美观满意度、远期后牙折裂情况。结果 修复后12个月,观察组修复优良率92.86%高于对照组的76.19%,差异有统计学意义(P<0.05)。修复后6个月,观察组修复成功率95.24%高于对照组的80.95%,差异有统计学意义(P<0.05)。修复后6个月,两组GI、SBI较修复前降低, PD、AL较修复前减少,且观察组GI(0.22±0.06)分、SBI(0.38±0.13)低于对照组的(0.30±0.07)分、(0.59±0.12), PD(1.31±0.23)mm、AL(0.31±0.07)mm少于对照组的(1.97±0.28)、(0.58±0.05)mm,差异有统计学意义(P<0.05)。修复后12个月,观察组美观满意度为88.10%,高于对照组的69.05%,差异有统计学意义(P<0.05)。修复后24个月,观察组的后牙折裂率为7.14%(3/42),低于对照组的23.81%(10/42),差异有统计学意义(P<0.05)。结论 对于行后牙冠修复的患者,在E-Max玻璃陶瓷高嵌体的辅助治疗下能够提高美观满意程度、修复效果以及修复成功的几率,同时还可降低患者远期后牙的折裂率,改善牙周状况。Objective To discuss the effects of E-Max glass-ceramic onlays on periodontal index and long-term posterior tooth fracture rate in patients undergoing posterior crown restoration. Methods A total of 84 patients(84 affected teeth) were divided into control group and observation group according to the random numerical table, with 42 cases(42 affected teeth) in each group. The control group received conventional cobaltchromium porcelain crowns for restoration, and the observation group received E-Max glass-ceramic onlays for restoration. Both groups were compared in terms of restoration effect, restoration success rate, gingival index(GI),sulcus bleeding index(SBI), pocket depth(PD), attachment loss(AL) level, aesthetic satisfaction, and long-term posterior tooth fracture rate. Results At 12 months after restoration, the excellent and good rate of restoration in the observation group was 92.86%, which was higher than 76.19% in the control group, and the difference was statistically significant(P<0.05). At 6 months after restoration, the restoration success rate in the observation group was 95.24%, which was higher than 80.95% in the control group, and the difference was statistically significant(P<0.05). At 6 months after restoration, the GI and SBI in the two groups were lower than those before restoration in this group, and PD and AL were less than those before restoration in this group;the GI(0.22±0.06) and SBI(0.38±0.13) in the observation group were lower than(0.30±0.07) and(0.59±0.12) in the control group;the PD(1.31±0.23) mm and AL(0.31±0.07) mm in the observation group were lower than(1.97±0.28) and(0.58±0.05) mm in the control group;the differences were statistically significant(P<0.05).
