摘 要:目的研究在骨增量中利用改良引导骨再生(GBR)技术的临床价值。方法21例骨增量手术患者均利用改良GBR技术开展手术操作,统计分析手术结果、并发症情况及各时间点测量结果。结果21例患者共计26个位点接受改良GBR手术,术后所有伤口均愈合良好,未发生感染及伤口大面积裂开、植骨材料流失情况。每个位点都获得不同程度的骨宽度增量(1.799±1.454)mm~(3.187±1.557)mm,每个位点都出现相似的骨宽度丧失量(0.868±0.745)mm~(1.385±0.778)mm。牙槽嵴顶(R0)、牙槽嵴顶下2 mm(R2)、牙槽嵴顶下4 mm(R4)、牙槽嵴顶下6 mm(R6)、牙槽嵴顶下8 mm(R8)处的牙槽骨宽度在手术完成6个月后(T2)时为(4.742±1.276)、(7.390±1.301)、(9.260±1.371)、(10.167±1.665)、(10.523±2.162)mm,明显大于术前(T0)时的(2.943±1.202)、(4.828±1.431)、(6.072±1.847)、(7.157±2.412)、(8.215±2.534)mm,差异有统计学意义(P<0.05)。R0、R2、R4、R6处的牙槽骨宽度在手术完成后当日(T1)时为(5.610±1.435)、(8.775±1.217)、(10.437±1.453)、(11.201±1.617)mm,明显大于T2时的(4.742±1.276)、(7.390±1.301)、(9.260±1.371)、(10.167±1.665)mm,差异有统计学意义(P<0.05);R8处的牙槽骨宽度在T1时大于T2时,但差异无统计学意义(P>0.05)。结论在骨增量中利用改良GBR技术有较好的手术效果。Objective To study the clinical value of modified guided bone regeneration(GBR)technique in bone augmentation.Methods 21 patients with bone augmentation surgery were operated with modified GBR technique,and the surgical outcome,complications and measurements at each time point were analyzed statistically.Results A total of 26 sites in 21 patients underwent modified GBR surgery,and all wounds healed well after surgery without infection or extensive wound dehiscence or loss of bone graft material.Different degrees of bone width increment of(1.799±1.454)mm to(3.187±1.557)mm were obtained at each site,and similar bone width loss of(0.868±0.745)mm to(1.385±0.778)mm occurred at each site.The alveolar bone width at alveolar crest(R0),2 mm below alveolar crest(R2),4 mm below alveolar crest(R4),6 mm below alveolar crest(R6),8 mm below alveolar crest(R8)were(4.742±1.276),(7.390±1.301),(9.260±1.371),(10.167±1.665),(10.523±2.162)mm at 6 months after surgery(T2),which were significantly greater than(2.943±1.202),(4.828±1.431),(6.072±1.847),(7.157±2.412),(8.215±2.534)mm at the time before surgery(T0),and the differences were statistically significant(P<0.05).The alveolar bone width at R0,R2,R4,and R6 were(5.610±1.435),(8.775±1.217),(10.437±1.453),(11.201±1.617)mm on the day(T1)after surgery,which were significantly greater than(4.742±1.276),(7.390±1.301),(9.260±1.371),and(10.167±1.665)mm at T2,and the differences were statistically significant(P<0.05).The alveolar bone width at R8 was greater at T2 than that at T1,but the difference was not statistically significant(P>0.05).Conclusion The modified GBR technique has a good effect in bone augmentation.
