摘 要:目的:探讨双源CT(DSCT)在鉴别诊断肾细胞癌不同病理类型中的应用价值。方法:选择2019年10月-2021年10月厦门大学附属第一医院收治的62例肾细胞癌患者作为观察对象,均经肾穿刺活检、手术病理学确诊。62例患者均行双源CT检查,记录肾细胞癌的病理类型及细胞排列结构,分析不同病理类型肾细胞癌患者的DSCT扫描征象。结果:不同病理类型肾细胞癌的DSCT平扫密度、增强扫描强化程度对比,差异无统计学意义(Z=1.906、2.514,P=0.592、0.473)。不同排列结构的肾细胞癌DSCT平扫密度对比,差异无统计学意义(Z=4.782,P=0.188);不同排列结构的肾细胞癌增强扫描强化程度对比,差异有统计学意义(Z=18.486,P=0.000);肾透明细胞癌患者的皮质期、实质期DSCT值均高于其他类型肾细胞癌,差异有统计学意义(P<0.05);肾透明细胞癌的均匀强化方式占比低于其他亚型肾细胞癌,不均匀强化方式占比高于其他亚型肾细胞癌,差异有统计学意义(P<0.05)。结论:双源CT在肾细胞癌不同病理类型鉴别诊断中应用价值高,通过分析双源CT多期增强扫描的征象能对肾细胞癌不同病理类型的特征进行较为直观地反映,为临床制定干预措施提供参考依据。Objective:To investigate the value of dual source CT(DSCT)in differential diagnosis of different pathological types of renal cell carcinoma.Method:A total of 62 patients with renal cell carcinoma admitted to the First Affiliated Hospital of Xiamen University from October 2019 to October 2021 were selected as observation objects,all of whom were confirmed by renal puncture biopsy and surgical pathology.Sixty-two patients were examined with dual source CT.The pathological types and cell arrangement of renal cell carcinoma were recorded,and the DSCT scanning signs of different pathological types of renal cell carcinoma were analyzed.Result:There were no significant difference in DSCT plain scan density and enhanced scan intensity among different pathological types of renal cell carcinoma(Z=1.906,2.514;P=0.592,0.473).There was no significant difference in DSCT plain scan density of renal cell carcinoma with different structures(Z=4.782,P=0.188).The enhancement degree of renal cell carcinoma with different arrangement structures was statistically significant differences(Z=18.486,P=0.000).The DSCT values of renal clear cell carcinoma patients in cortex and parenchyma were higher than those of other types of renal cell carcinoma,the differences were statistically significant(P<0.05).The proportion of homogeneous enhancement mode in clear cell renal carcinoma was lower than that in other subtypes of renal cell carcinoma,and the proportion of heterogeneous enhancement mode was higher than that in other subtypes of renal cell carcinoma,with statistically significant differences(P<0.05).Conclusion:Dual source CT has high application value in the differential diagnosis of different pathological types of renal cell carcinoma.The characteristics of different pathological types of renal cell carcinoma can be directly reflected by analyzing the signs of dual source CT multi-phase enhanced scanning,which provides a reference for clinical intervention.
