改良重睑切口额肌瓣悬吊术矫正儿童重度先天性上睑下垂的临床体会

摘　　要：目的:探讨改良重睑切口额肌瓣悬吊术矫正儿童重度先天性上睑下垂的临床效果。方法:选取2015年2月-2022年1月莆田学院附属医院58例(74眼)先天性上睑下垂患儿为研究对象进行回顾性研究,所有患儿均给予全麻下手术,将行改良重睑切口额肌瓣悬吊术的29例(37眼)纳入观察组,将行传统双切口额肌瓣悬吊术的29例(37眼)纳入对照组。术后随访3~12个月,观察两组手术矫正效果和并发症发生率。结果:观察组术后总矫正率为100%(37/37),高于对照组的86.49%(32/37),差异有统计学意义(P<0.05)。观察组术后并发症发生率为10.81%(4/37),低于对照组的35.14%(13/37),差异有统计学意义(P<0.05)。随访期间,未发现感染或肉芽肿等并发症。结论:改良重睑切口额肌瓣悬吊术有效保证了儿童重度先天性上睑下垂的矫正效果,并降低了并发症发生率,且患儿术后眼睑弧度美观自然,是比较理想的手术方式。Objective:To investigate the clinical effect of modified double eyelid incision frontalis muscle flap suspension operation in correction of severe congenital ptosis in children.Method:Fifty-eight children(74 eyes)with congenital ptosis in the Affiliated Hospital of Putian University from February 2015 to January 2022 were selected for retrospective study.All children were operated under general anesthesia,and 29 cases(37 eyes)who underwent modified double eyelid incision frontalis muscle flap suspension operation were included in the observation group,29 cases(37 eyes)who underwent traditional double-incision frontalis muscle flap suspension operation were included in the control group.The postoperative followed-up for 3 to 12 months,and the surgical correction effect and postoperative complication rate of two groups were observed.Result:The total correction rate of the observation group after operation was 100%(37/37),which was higher than 86.49%(32/37)of the control group,the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 10.81%(4/37),which was lower than 35.14%(13/37)in the control group,and the difference was statistically significant(P<0.05).During followed-up,no complications such as infection or granuloma was found.Conclusion:Modified double eyelid incision frontalis muscle flap suspension operation effectively guarantees the correction of severe congenital ptosis in children,and reduces the incidence of complications,and the eyelid curvature is beautiful and natural after surgery,which is an ideal surgical method.
