摘  要:目的:探讨肠道菌群特征、叶酸与首发未用药精神分裂症(SCZ)患者临床症状的相关性,并分析预测SCZ发生的价值。方法:选取2021年5月-2022年4月常州市德安医院SCZ患者86例,设为SCZ组,另选取同期体检健康志愿者60例,设为对照组。比较两组肠道菌群含量及相关指标叶酸水平,观察SCZ患者发作期、缓解期临床症状阳性与阴性症状量表(PANSS)评分变化,采用Pearson相关分析肠道菌群相关指标、叶酸与SCZ患者发作期PANSS评分的关系,建立ROC曲线分析叶酸、肠道菌群含量预测SCZ发生的价值。结果:SCZ组叶酸水平、双歧杆菌含量均低于对照组,拟杆菌属含量高于对照组,差异均有统计学意义(P<0.05)。SCZ患者缓解期PANSS总分、分项分及补充项分较发作期均下降(P<0.05)。SCZ患者叶酸水平与双歧杆菌含量呈正相关(r=0.524,P=0.000),与拟杆菌属含量呈负相关(r=-0.476,P=0.000);叶酸水平、双歧杆菌含量与发作期PANSS总分、分项分及补充项分均呈负相关(P<0.05);拟杆菌属含量与发作期PANSS总分、分项分及补充项分均呈正相关(P<0.05)。ROC分析显示,叶酸、双歧杆菌、拟杆菌属、双歧杆菌+拟杆菌属及三项联合预测SCZ发生的AUC分别为0.775、0.772、0.729、0.875、0.934,取最佳界值时,单独指标敏感度偏低,两项联合、三项联合敏感度明显提高,分别为0.783、0.833。结论:首发未用药SCZ患者叶酸、肠道菌群含量变化明显,与临床症状严重程度密切相关。叶酸、双歧杆菌含量、拟杆菌属含量联合预测SCZ发生价值较高。Objective:To investigate the correlation between intestinal flora characteristics,folic acid and clinical symptoms of first-episode untreated schizophrenia(SCZ)patients,and to analyze the value of predicting the occurrence of SCZ.Method:A total of 86 patients with SCZ in Changzhou De’an Hospital from May 2021 to April 2022 were selected as SCZ group,and 60 healthy volunteers who underwent physical examination during the same period were selected as the control group.The content of intestinal flora and level of folic acid were compared between two groups,and the changes of positive and negative syndrome(PANSS)score of clinical symptoms in the stage of attack and remission of SCZ patients were observed.Pearson correlation analysis was used to analyze the relationship between intestinal flora related indicators,folic acid and PANSS score of SCZ patients,and ROC curve was established to analyze the value of folic acid and intestinal flora content in predicting the occurrence of SCZ.Result:The folic acid level and Bifidobacterium content in SCZ group were lower than those in the control group,while Bacteroidetes content was higher than that in the control group,the differences were statistically significant(P<0.05).The PANSS total score,component score and supplementary score of SCZ patients in remission stage were significantly lower than those in attack stage(P<0.05).There was a significant positive correlation between folic acid level and bifidobacterium content in SCZ patients(r=0.524,P=0.000),and a significant negative correlation between folic acid level and Bacteroidetes content(r=-0.476,P=0.000).The levels of folic acid and Bifidobacterium in SCZ patients were significantly negatively correlated with the total score,component score and supplementary score of PANSS(P<0.05);the content of Bacteroidetes were significantly positively correlated with the total score,component score and supplementary score of PANSS in attack stage(P<0.05).ROC analysis showed that AUC of folic acid,Bifidobacteria,Bacteroides,Bifido
