摘　　要：目的 探讨卵巢癌患者血清糖类抗原(CA)125、CA153、CA19-9水平与临床意义。方法 将213例卵巢占位性病变患者根据病理结果分为病例组(卵巢癌)107例和对照组(卵巢良性病变)106例。比较两组患者的临床特征和血清CA125、CA153、CA19-9水平;比较不同国际妇产科联盟(FIGO)分期卵巢癌患者血清CA125、CA153、CA19-9水平;比较血清CA125、CA153、CA19-9单独及联合检测对卵巢癌的诊断价值。结果 病例组患者血清CA125、CA153、CA19-9水平均明显高于对照组,差异均有统计学意义(P﹤0.01)。Ⅲ~Ⅳ期卵巢癌患者血清CA125、CA153、CA19-9水平均明显高于Ⅰ~Ⅱ期患者,差异均有统计学意义(P﹤0.01)。CA125、CA153与CA19-9均对卵巢癌有一定的诊断价值,而三者联合检测诊断卵巢癌的灵敏度高于三者单独检测。病理诊断Ⅰ~Ⅱ期卵巢癌48例,CA125、CA153与CA19-9联合检测诊断Ⅰ~Ⅱ期卵巢癌40例,三者联合检测诊断Ⅰ~Ⅱ期卵巢癌的灵敏度为0.83(40/48)。结论 血清CA125、CA153与CA19-9联合检测对卵巢癌及其分期有较高的诊断价值。Objective To investigate the levels and clinical significance of serum carbohydrate antigen(CA)125,CA153,and CA19-9 in patients with ovarian cancer.Method A total of 213 patients with ovarian space-occupying lesions were divided into case group(ovarian cancer)with 107 cases and control group(ovarian benign lesions)with 106cases according to the pathological examination.The clinical significance and serum CA125,CA153,and CA19-9 levels of the two groups of patients were compared.The serum CA125,CA153,and CA19-9 levels of ovarian cancer patients with different International Federation of Gynecology and Obstetrics(FIGO)stages were compared.The diagnostic value of serum CA125,CA153,and CA19-9 alone and combined detection for ovarian cancer were also compared.Result The serum levels of CA125,CA153,and CA19-9 in the case group were significantly higher than those in the control group,and the differences were statistically significant(P<0.01).The levels of serum CA125,CA153,and CA19-9 in patients with stageⅢ-Ⅳovarian cancer were significantly higher than those in patients with stageⅠ-Ⅱ,and the differences were statistically significant(P<0.01).CA125,CA153,and CA19-9 all had certain diagnostic value for ovarian cancer,and the combined detection of the three could achieve higher sensitivity than that of any of the three alone.The combined detection of CA125,CA153,and CA19-9 could diagnose 40 cases of stage I-II ovarian cancer whereas the pathological diagnosis of 48 cases.And the sensitivity of the combined detection for the diagnosis of stageⅠ-Ⅱovarian cancer was 0.83(40/48).Conclusion The combined detection of serum CA125,CA153,and CA19-9 has high diagnostic value for ovarian cancer and its staging.
