宫颈环形电切术治疗早期宫颈癌的临床疗效

摘　　要：目的探讨宫颈环形电切术治疗早期宫颈癌的临床疗效。方法根据手术方式的不同将127例早期宫颈癌患者分为观察组(n=66,采用宫颈环形电切术)和对照组(n=61,采用宫颈冷刀锥切术)。比较两组患者的鳞状细胞癌抗原(SCC-Ag)水平、围手术期指标、复发率、人乳头瘤病毒(HPV)DNA转阴率以及并发症发生率。结果手术后,两组患者的SCC-Ag水平均低于本组手术前,且观察组患者的SCC-Ag水平低于对照组,差异均有统计学意义(P<0.05)。观察组患者切除宫颈组织宽度明显小于对照组,手术时间和住院时间均明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P<0.01)。两组患者的复发率、HPV DNA转阴率比较,差异均无统计学意义(P>0.05)。观察组患者的并发症总发生率低于对照组(P<0.05)。结论宫颈环形电切术治疗早期宫颈癌患者可降低SCC-Ag水平,使HPV DNA转阴,其手术时间短,术中出血量少,有利于患者术后恢复,且安全性较高,可考虑在临床中推广应用。Objective To investigate the clinical efficacy of loop electrosurgical excision procedure in the treatment of early cervical cancer.Method A total of 127 patients with early cervical cancer were divided into observation group(n=66,receiving loop electrosurgical excision procedure)and control group(n=61,receiving cervical cold knife conization)according to different surgical methods.The squamous cell carcinoma antigen(SCC-Ag)level,perioperative indicators,recurrence rate,human papillomavirus(HPV)DNA negative conversion rate,and complication rate were compared between the two groups.Result After the operation,the SCC-Ag levels of the two groups were lower than those before the operation,and the SCC-Ag level of the observation group was lower than that of the control group,and the differences were statistically significant(P<0.05).The width of resected cervical tissue in the observation group was significantly smaller,the operation time and hospitalization time were significantly shorter,and the intraoperative blood loss was significantly less than those in the control group,and the differences were statistically significant(P<0.01).There were no significant differences in the recurrence rate and HPV DNA negative conversion rate between the two groups(P>0.05).The total incidence of complication in the observation group was lower than that in the control group(P<0.05).Conclusion Loop electrosurgical excision procedure for patients with early cervical cancer could down-regulate the level of SCC-Ag and accelerate HPV DNA negative.It is beneficial to the postoperative recovery of patients and has high safety because of its short operation time and less intraoperative blood loss,so it could be considered for clinical application.
