痛泻要方加味联合隔药灸治疗肝气乘脾证腹泻型肠易激综合征临床研究

摘　　要：目的:观察痛泻要方加味联合隔药灸治疗肝气乘脾证腹泻型肠易激综合征(IBS-D)的临床疗效及其对炎症因子水平的影响。方法:将100例肝气乘脾证IBS-D患者随机分为对照组和观察组各50例。对照组给予双歧杆菌三联活菌散口服治疗,观察组给予痛泻要方加味联合神阙穴隔药灸治疗。治疗6周后,观察比较2组临床疗效、IBS病情严重程度调查表(IBS-SSS)积分、中医证候积分及血清炎症因子[白细胞介素-6 (IL-6)、肿瘤坏死因子-α (TNF-α)]水平。结果:观察组总有效率为84.00%,对照组为66.00%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组IBS-SSS积分及中医证候积分比较,差异无统计学意义(P>0.05);治疗后,2组IBS-SSS积分及中医证候积分均较治疗前下降(P<0.05),且观察组2项积分均低于对照组(P<0.05)。治疗前,2组IL-6、TNF-α水平比较,差异无统计学意义(P>0.05);治疗后,2组IL-6、TNF-α水平均较治疗前下降(P<0.05),且观察组2项指标均低于对照组(P<0.05)。结论:痛泻要方加味联合隔药灸治疗肝气乘脾证IBS-D可提高治疗效果,改善临床症状和中医证候,并减轻机体炎症反应。Objective: To observe the clinical effect of modified Tongxie Yaofang combined with moxibustion on medicinals for diarrhea-predominant irritable bowel syndrome(IBS-D) with the liver qi attacking the spleen syndrome and its effects on inflammatory factors. Methods:A total of 100 cases of IBS-D patients with liver qi attacking the spleen syndrome were randomly divided into the control group and the observation group,with 50 cases in each group. The control group was given the oral administration of live combined bifidobacterium, lacto bacillus and enterococcus powder, and the observation group was treated with modified Tongxie Yaofang combined with moxibustion on medicinals at Shenque point(RN 8).After six-week treatment,clinical effects,scores of Irritable Bowel Syndrome Severity Scoring System(IBS-SSS),traditional Chinese medicine(TCM) syndrome scores,and levels of serum inflammatory factors including interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α) in the two groups were observed and compared. Results:The total effective rate was 66.00% in the control group and 84.00% in the observation group, the difference being significant(P<0.05). Before treatment, there was no significant difference being found in the comparison of IBS-SSS scores and TCM syndrome scores between the two groups(P>0.05). After treatment,IBS-SSS scores and Chinese medicine syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05). Before treatment,there was no significant difference being found in the comparison of levels of IL-6 and TNF-α between the two groups(P>0.05). After treatment,levels of IL-6 and TNF-α in the two groups were decreased when compared with those before treatment(P<0.05),and the two levels in the observation group were lower than those in the control group(P<0.05).
