摘 要:目的:探讨脑CTP成像中不同重建时间间隔对灌注参数及脑梗死诊断准确性的影响。资料与方法:收集临床诊断为急性缺血性脑卒中的患者25例,所有患者均利用256层螺旋CT(Revolution,GE healthcare)进行头颅CT灌注成像,扫描间隔时间为1.5s,扫描循环25期,每组扫描时间0.5s,采用CT perfusion灌注软件进行CTP图像后处理分析,第1组图像为标准重建图像,选择全期图像(25期,扫描时间0.5s,间隔1.5s)进行图像后处理,第2组重建图像选择剔除每两组中间一组图像进行后处理(13组,扫描时间0.5s,间隔3.5s),经过后处理得到脑组织CT灌注伪彩图,测量两组后处理图像梗死区及对侧正常脑组织各灌注参数值,比较两组重建图像得到的各灌注参数值的差异及医师对脑梗死诊断准确性的差异。结果:在两组后处理重建图像中,梗死区测量的灌注参数CBV、CBF、MTT、TTP、Tmax差异均无统计学意义(P>0.05),两组图像对侧正常脑实质的灌注参数CBV、CBF、MTT、TTP、Tmax差异亦均无统计学意义(P>0.05)。医师在第1组重建图像下诊断脑梗死25例,在第2组重建图像下诊断脑梗死24例,两组差异无统计学意义(P>0.05)。结论:适当增加重建时间间隔及减少重建扫描次数对脑CTP灌注参数无明显影响,对脑梗死诊断的准确性亦无明显影响,在脑CTP成像中,可以适当增加扫描时间间隔及减少扫描次数。Objective:To investigate the influence of different reconstruction time intervals in brain CTP imaging on perfusion parameters and the accuracy of cerebral infarction diagnosis.Methods:A total of 25 patients who were clinically diagnosed as acute ischemic stroke were collected.All patients were used 256-slice spiral CT(Revolution,GE healthcare) for head CT perfusion imaging.The scan interval was 1.5 s,and the scan cycle was 25 phases,The scanning time of each group is 0.5 s,and the CT perfusion software is used for CTP image post-processing analysis.The first group of images is the standard reconstructed image.Select the full-period image(25 periods,scanning time 0.5 s,interval 1.5 s) for image post-processing Processing,the second group of reconstructed images is selected to eliminate the middle group of images in each two groups for post-processing(13 groups,scanning time 0.5 s,interval 3.5 s),after post-processing,the brain tissue CT perfusion pseudo-color map is obtained.After measuring the two groups The perfusion parameter values of the infarct area and the contralateral normal brain tissue were processed,and the differences in the perfusion parameter values obtained from the two groups of reconstructed images and the differences in the accuracy of the diagnosis of cerebral infarction by the physician were compared.Results:In the reconstructed images of the two groups,the perfusion parameters CBV,CBF,MTT,TTP,Tmax measured in the infarct area were not statistically different(P>0.05).The differences in perfusion parameters CBV,CBF,MTT,TTP and Tmax were also not statistically significant(P>0.05).The doctor diagnosed 25 cases of cerebral infarction under the reconstructed images in the first group,and diagnosed 24 cases of cerebral infarction under the reconstructed images in the second group.There was no significant difference between the two groups(P>0.05).Conclusion:Appropriately increasing the reconstruction time interval and reducing the number of reconstruction scans have no significant impact on cerebral
