摘　　要：目的:探究来那度胺联合VD方案对多发性骨髓瘤(MM)患者T淋巴细胞亚群水平的影响。方法:回顾性分析2017年1月—2020年1月开封市人民医院收治的20例MM患者临床资料,将使用来那度胺联合VD方案治疗的10例患者纳入观察组,将使用VD方案治疗的10例患者纳入对照组,两组患者均治疗4个周期。比较两组患者治疗前、治疗4个周期后血红蛋白(Hb)、骨髓浆细胞及T淋巴细胞亚群[CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)]。结果:治疗前,两组患者Hb、骨髓浆细胞及CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平对比,差异无统计学意义(P>0.05);治疗4周期后,两组患者Hb含量及CD4^(+)、CD4^(+)/CD8^(+)水平均升高,骨髓浆细胞及CD8^(+)水平均降低,且观察组变化幅度更大,差异有统计学意义(t=6.018、4.008、15.821、2.723、3.682,P<0.05)。结论:MM患者采用来那度胺联合VD方案治疗可以降低骨髓病态浆细胞含量,改善血红蛋白水平,纠正T淋巴细胞紊乱,增强机体免疫功能。Objective:To investigate the effect of lenalidomide combined with VD regimen on the level of T lymphocyte subsets in patients with multiple myeloma(MM).Methods:The clinical data of 20 patients with MM admitted to the hospital from January 2017 to January 2020 were retrospectively analyzed.Ten patients treated with lenalidomide combined with VD regimen were included in the observation group,and 10 patients treated with VD regimen were included in the control group,and both groups were treated for 4 cycles.The hemoglobin(Hb),bone marrow plasma cells and T lymphocyte subsets(CD4^(+),CD8^(+),CD4^(+)/CD8^(+))were compared between the two groups before and after 4 cycles of treatment.Results:Before treatment,there was no statistically significant difference in Hb,bone marrow plasma cells and CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)levels between the two groups(P>0.05).After 4 cycles of treatment,Hb content and CD4^(+)and CD4^(+)/CD8^(+)levels increased in both groups,and bone marrow plasma cells and CD8^(+)levels decreased in both groups,and the change was greater in the observation group,with statistically significant differences(t=6.018,4.008,15.821,2.723,3.682,P<0.05).Conclusion:Treatment of MM patients with lenalidomide combined with VD regimen can reduce the level of bone marrow pathological plasma cells,improve the level of hemoglobin,correct T lymphocyte disorder and enhance the immune function of the body.
